"Received 1, you're going to have M-4 coming."

"Central to M-4, respond to 1070 Fourth Ave, M-1 is out with a seizure victim, requesting assistance. Nature of assistance unknown."

*I hear the tones and rush out to the truck*

"Medic 4 copies were enroute"

*I say as I flip on the lights and turn into traffic*

"Medic 4 to Medic 1: Can you advise what equipment you need us to bring up"

"Received 4."

" Medic-1 to 4 i need a stretcher and a trauma kit. "

"Medic 4 copies"

*I turn onto 4th avenue and pull up behind Medic-1*

"Medic 4 is on scene"

*I grab the trauma kit and assist my partner in pushing the cot inside*

'Hey, Jamie what do you need me to do?'

*I say putting the trauma kit on the ground*

* I push another 5mp of Diazepam through the IV and the seizure stops. the patients mouth begins to loosen but she begins to Vomit. *

' Keep her on her side so she doesnt aspirate. '

" Medic 1 to Medic 4 Seizure has stopped however patient is Vomiting now. "

'Affirm, Jamie do you have suction kit with you?'

* I look up and see Lafferty and his partner walking in with the stretcher. *

' Whew its good to see you guys, can you start another IV with normal Saline and start o2 with Nasal canula and yes the suction kit should be in the ALS bag. '

'Gotcha'

*I start another IV with normal Saline*

*I have my partner start the patient on O2 4l/m via NC and get the suction kit*

'Jamie were ready whenever you are. And you can call me Dave if you want'

*I say trying to lighten the mood some*

' Thx dave, could we get 1 more set of vitals before we transport. '

'Sure thing'

*I adjust the BP cuff on the patients left arm and pump it up and deflate it.*

'BP: 140/90 P: 110 Resp: 24/rapid Pulse Ox: 98%'

*ACTIVE STANDBY DUTY ALARM*

' alright Dave lets go ahead and transport her. '

* Dave, Vaughn and I load her onto the stretcher and guide her out of the house, down the stairs and out to the ambulance. I look over at her father *

' Sir do you want to ride with us or take your own car?. '
Father: No I will take my car.
Me: Ok she will be going to ERT Medical Center

* Dave and vaughn load her into my Ambulance . *

' Dave can you ride in back? or do you want meto ride in back?. '

'Hey Vaughn take Medic-4 and meet us at the Hospital'

'How about we both ride in back incase she seizes again.'

'Better to have an extra set of hands back there'

*I say trying to sounds as cheerful as possible*

' Then who is gonna drive the ambulance then lol. Zach is on leave. And we best hurry, Something has happened, did u get those tones. Either way lets roll code. '

'Oh I didn't hear about him being on leave'

'I'll drive then since you've already been giving primary treatment'

*I say as I hop in the drivers seat and pick up the mic*

"Medic 1: Code 3 to ETC"

*I then put the truck in gear and turn the siren on as I start toward the hospital*

'Jamie how are you doing back there'

*I say as I make a I make a left hand turn*

'We are about 2 mins out'

*I have to drive most of the way in opposing lanes because stupid people won't get out of the way*

'Alright Jamie were pulling into the complex'

*I say as I shut off the siren and see Medic 4 turning in behind us*

*I then back the rig up and pick up my radio*

"Medic 1 is at destination ETC"

*And then go with Richardson who pulled up and open the rear doors and help Jamie wheel the patient into the ER*

* stepping into the ambulance unit for the 1st time since my brief absence feels weird. but I still get in and close the door, turn it on and pick up the radio. *

" Central Medic 1 available on the radio. "

* I drive out of the ambulance bay and out into traffic. *

"M-1 please return to quarters to pick up your new partner, McGuire"

" Copy that COPA Medic 1 returning to Hospital. "

"Central to M-1. Respond to Kitson Elementary School, in the cafeteria, for a child choking. Object has been dislodged but the nurse is requesting you come check him out."

EAT:
M-1-Kitson Elementary, 8 y/o coking on food, item is dislodged.

"EMS-1 received, Kitson Elementary."

*I walk out to the bay, stepping into the drivers seat of M1. I start the engine as Jaime gets in, I pull out, press the garage door button to close the door behind me, and as I drive down the driveway, I push the 'Master' button to turn on the lights. I hit the Manual button on the siren a few times as I pull into traffic and respond to Kitson.*

*I pull into the school parking lot, stop in front of the walkway to the door, press the anti-theft button and take the keys out, clipping them to a belt loop.*

"EMS-1 is out at Kitson."

'Jaime, you wanna grab the chart? From now on just switch off between doing the chart and evaluation each call?'

*I grab the jumpbag, and walk up to the front door. I attempt to open it, but it is locked. I press the call button next to the door.*

'Hello?'

'ERT-EMS, we're here for a child choking.'

'Take a left and the cafeteria is straight ahead.'

*The door clicks, I open in, holding it for Jaime, and walk down the hall. I see the nurse and two other adults kneeling around a crying child. I walk up and kneel next to him.*

'Hey buddy. What's wrong?'

*crying* (... indicated pauses for sniffling) 'I...I was choking...on my food...I count breath.'

*I look up at and adult*

'Can we get a chair?'

*An adult nods his head and walks into a nearby classroom and brings out a chair*

'Take a seat over okay? What's your name?'

*He sits down*

'Zach.'

'Alright Zach, *I get a stethoscope out of the jump bag* I'm going to listen to your breathing okay? Just like at the doctor.'

*I listen to his breathing, other than the crying, it sounds normal and unobstructed*

*I stand up*

Me 'Are one of you the principal?'

Person 'I am'

*He holds out his hand, I shake it*

'Hi, do you have an emergency card for Zach? If you do give it to Paramedic Morrison here, and also call his parents.'

Principal 'Oh, sure.'
*The principal walk into the office.*

"Received EMS-1."

(Since when are you EMS here?)

* I grab the chart and start taking down notes given to me from the principal. *

' does he have any known allergies? any kind of breathing issues asthma anything like that? '
Principal: No not that i know of?

Me: ok no worries we're gonna check him out here and see, he may need to be checked out at the hospital but other than that he Should be ok. '

* I lean over to doug and see that he is examining the patient. *

' Hey Doug find out if he has any kind of a sore throat? '

* I look over at the Principal who is on the phone to the boy's parents and is visibly shaken *

' If they need to talk to me they can, if it makes them feel better. '

*I take out a flash light and turn it on shining it into the boy's throat.*

'Say ahh.'

'Ahhh.......'

*There doesn't appear to be any soreness*

'I don't see any redness or anything looking sore.'

*I look back at Zach.*

'Does your throat hurt or feel sore?'

'It did earlier, when I was choking but not now.'

'Zack, do you know what you were choking on?'

'A peanut.'

'Were they yours or a friends?'

'They were mine.'

'Okay, you're not allergic or anything?'

'I'm allergic to pollen.'

'Okay, sit tight for a second.'

*I stand up and face Jaime and the Principal.*

'We're going to need parental consent for sure, so why don't you have them come to the school to see Zack. I don't think he needs to go to the hospital, his breathing sounds fine, there doesn't appear to be any obstructions or injuries. But we'll need to speak to the parents, they have the final word.'

' Yea i agree he doesnt need to go to the hospital but if the parents insist we can leave the paperwork with the Principal and clear and a BLS unit can take over from here. unless u would like to wait? '

* I turn back to Zack and Wink *

*I turn to the principal*

'Is somebody on the way?'

'His mother is on her way from home.'

'Well that can't be too far away. Let's stay just so that we can talk to her and so we don't tie up a city unit.'

*After a few minutes a frantic mother comes running in.*

'Are you Zack's mother?'

'Yes, what's wrong?'

'We were called her because he was choking on something at lunch. He told us it was a peanut but it was dislodged. I listened to his breathing everything sounded clear. There was no soreness in his throat, and just by looking at him he's definitely gotten better between now and when we arrived. Would you like him to go to the hospital to be checked out?'

'Well, you're the professional, what do you think?'

'Honestly, I don't think he needs to. As I said everything sounded clear.'

'Well, then no he doesn't have to. Maybe I'll just bring him home and give him the rest of the day off.'

'That's a good idea. If you notice anything unusual in his breathing or anything then please don't hesitate to call us.'

'Oh, don't worry, thank you very much.'

'No problem. My partner is just going to have you sign an RMA sheet. That stands for Refused Medical Attention, at basically says that you refused further medical treatment by us or the hospital. Like I said, don't hesitate to call back if there is anything unusual.'

*I kneel down by Zack.*

'Feel better, alright Buddy? Have a great day!'

*I hold out my hand for a high-five, he slaps it.*

'Ouch, that's a strong high five.'

*I put the stethoscope back in the jumpbag, stand up throwing it over my shoulder.*

'Jaime, I'm going to go out, just have her sign the sheets and all and I'll meet you outside.'

*I shake hands with the principal and nurse then walk out. As I exit school, and push the talk button on my radio.*

"EMS-1 to Central, clear of Kitson, parental RMA."

*I walk to the truck, put the jumpbag back in the cabinet, step into the drivers seat shutting the lights off, and turning on the radio to the local hits station.*

* I walk out a short time after Doug and step into the passenger's seat. *

" Central, Medic-1 clear available on radio. "

' Alright Doug where do u want to go now? '

"Received EMS-1, you're available."

'Well, I think I'm going to head home for the night, and I'm laving for vacation tomorrow night so I won't be in.'

*I drive back to the ERT complex.*

"EMS unit please respond to Phoxlind Grocers for an unconscious cashier. See EAT for details"

EAT
15 Y/O cashier, relieving for breaks, when she just passed out and collapsed, hitting her face off the register. Black eyes forming, bloody and slightly disconfigured nose. Possible break. First aider on scene. Parents notified, and will be at ERT hospital waiting arrival, manager will be out front waiting for the ambulance.

(Happened saturday at the grocery store I go to)

" Central Medic 1 Responding. "

* I Put the ambulance in gear and Pull out. Once i hit traffic i turn the lights and sirens on. *

* I pull up to the store and turn my siren off, there is several people outside the building waving me down. *

" Central, Medic 1 show me on scene. "

* I stop the ambulance, step out and walk to the back, Grab my ALS bag and Trauma kit and walk up to the Manager. *

' Hello IM Jamie Morrison, what happened? '

Manager: Well i really dont know. I called her register to tell her to go to break and next thing I know im getting frantic calls saying she collapsed.

Me: Ok well lets take a look, where is she?

Manager: I had her Moved to my office in the back, Customers were crowding around her and she was beginning to Panic.

Me: Alright.

*I check my cell phone to see an alert sent out for EMS within my area... Actually in the store. I leave my cart with my wife and walk from aisle 6 to the checkouts to help the girl, on my way, I call COPA*
'ERT COPA Direct line'
'Burke, it's me Riles, I was in aisle 6 at the grocery store, I'll be at the checkout in say a minute to help out. I've got no meds on me, and I'm in civies'
'Sure, thanks Jase'
'No problem, I see her there with the first aider, a fellow cashier'

'Hi, what's going on here'
'She just took a spill, sir can you just back up please'
*I pull out my wallet and show my badge and ID*
'I'm ERT, Deputy Chief EMS Jason Riles, lemme see what I can do, we've got an ambulance on it's way'
'Oh, of course, come on in sir'
'Hi, I'm Jason, ERT EMS, how's she doin?'
'She's a little sore, fading in and out, probably broke her nose' The first responder, name tag David says (Had to make Dave fit in there somewhere)
'Yeah, okay, lets not move her and keep the pressure on the nose, I'm gonna check for swelling and possible neck pain'
'She said she was feeling no pain before she drifted back out, she's been passing out for years now.. Docs don't know why'
'Okay, well at least we know it's something that's chronic.'
*I start feeling around her neck and face wearing the blue gloves from the first aid kit on the cash register keyboard*

* I walk up to the patient and see she is stil unconscious but breathing on her own, there is a tremendous amount of blood around her face and a woman is attempting to clear the blood away from her face, Her nose is almost pointing sideways. I see Chief Riles already there and providing assistance *

Hey Chief Fancy seeing you here.

* Just then the Patient wakes up Violently and begins to thrash around. *

' Wo wo wo settle down, settle down, You took a bad fall. Just relax. '

* She reaches up and touches her nose and begins to scream. *

' No no no no dont touch that and relax seriously take it easy and let me work on u. U had a bad fall but ur alright just sit tight. Whats ur name.

Patient: Meg.

Me: Ok Meg relax does it hurt anywhere but ur nose obviously and whatever u do DONT BLOW.

Meg: yes my forehead and my right arm right here.

* She points to her lower arm which looks a bit disfigured as well. *

' Ok just sit tight we're gonna hook u up. '

" Central Medic-1 can we get a PEMS unit or a Fire unit here for back of the Ambulance transport manpower, McGuire is on Vacation and im responding alone. "

* I quickly put a splint on her lower arm and attempt to keep her calm. *

Me: Meg how old are you?

Meg: 15 almost 16.

Me: Ok how long have u been having these little episodes for?

Meg: I dont know for a while now, where is my mom and dad?

Me: They are at the hospital, they will meet u there.

Meg: I have to go to the hospital???????

* She begins to panic again and begin to cry. *

Me: No No No Meg dont start that ok, you have a broken nose and a possible broken arm, yes ur gonna need to see a doctor for that. Just relax ok? serious, dont start all this.

Meg: Its just I have been to the hospital SO much lately and I dont like it there.

"That's a negative, Jason Riles tells me he's there with you, if he's there, he'll ride with you for assist. You two can figure out who is driving"
*I check the boards again, but there are almost no medical units in the city free right now. I wonder what's going on!*

Me: Meg do you feel dizzy or any Hot Flashes before you black out or is it that you are good 1 minute then the next you wake up on the floor?.

Meg: Yea that, I just wake up after its over

Me: Any history of Diabetic emergencies in ur Immediate family?

Meg: No none

Me: Ok, Do you think you can walk now if we helped u up?

Meg: Probably, but my whole head hurts now.

Me: Then we're not gonna stand u up then lol we'll get a stretcher in here.

Meg: No its ok i will stand up I dont want everybody to see me all laying down like that.

Me: Well 2 things, 1 I wouldnt lay u down with a broken nose like that and 2 i will sit u up on the Stretcher so u wont be laying flat ok??

Meg: o ok i guess so.

Me: Alrighty then brb, Chief can u watch her please.

* I stand up and walk over to the ambulance carrying the ALS bag and the Trauma kit. I put them in the back of the Ambulance and grab the Stretcher, I start walking it into the Store. *

" Central Medic 1 if you have not Dispatched the additional units yet then please cancel them at this time, if they are en-route then go ahead and continue them in. "

* As i say that I get the message from COPA about no additional units to be given. *

" COPA, Medic-1 copies No additional units to be given, Riles and I can handle then. "

* I walk into the store with the stretcher and walk up to Meg and Riles. *

' Well Meg, you ready to go for a ride? '

Meg: Do I have a choice?

Me: Nah no really * i say with a smile *

Meg: Ok then I guess im as ready as i can be.

*I text my wife telling her I've been called to work and I'll see her in about 20 minutes*
'Alright, you ready Morrison?'

* I put the stretcher in the sitting position then lower it ot ground level. I get under the left arm of Meg and Riles takes the right *

' Yep ready Chief. '

* We lift her to her feet and get her to sit down on the stretcher, I strap her in and start walking her to the Ambulance with Riles pushing the stretcher. *

* We load the patient into the back of the ambulance and I climb in the back, Chief Riles closes the back doors and gets in the front to drive. *

" Central, Medic-1 we are transporting 1 patient Code 1 to ETC. "

"Understood, Riles, check in with front desk for someone to bring you home after you get here"

"Central to EMS-1. Number 2780 Chevy Road, Building 2 B - Bravo. For a person fallen down the stairs, reporting leg injury."

*EAT
EMS-1, 37 y/o female, tripped down flight of stairs 1-story high, reporting leg injury at the bottom. Patient Conscious and alert, coworkers on scene with her.

"EMS-1 received, 2780 Chevy Road, building Bravo 2."

*I walk out of the academy, get in the ambulance. I start the engine as Jaime gets in. I push the master button to turn on all the light. Hit the siren a few times as we pull onto the road and drive towards Chevy.*

*I pull into the lot, and I get greeted by somebody at the front door. I press the anti-theft button, grab the chart and step out.*

'Hey, what's going on here?'

'Follow me, she's in the back. They say she fell dwon the stairs, but the people who saw it would be able to tell you better than me. I just work the front desk.'

'Alright, just bring us back.'

*I follow her through a door, holding it open for Jaime, and we reach the woman on the ground.*

'Hey, what happened here?'

'I just lost my footing on the stairs. Bu I definitely did something to my leg, I can't even move it.'

'Alright, my partner is going to evaluate you and your injuries, can I have your name please?'

'Judy Smith.'

'Alright, common spelling?'

'Yes.'

*I write the name on the chart, and write down injuries, and cause of injury*

'What's your date of birth?'

'October 7, 1974.'

*I write 10/7/1974 on the chart.*

'Who is your doctor?'

'Doctor Jack Robertson. He's in Trulit.'

*I write the doctor in the box on the chart*

"Recieved, 1."

*The 911 phone rings, I answer and type info into the eat.
*I look at the board of available EMS units

"Central to EMS-5. 205 Fourth Avenue, apartment 3, second floor. Lift assist."

*EAT
EMS-5, 205 fourth Avenue, apt. 3, division 2, 67 y/o male fell out of bed. No injuries, needs assistance getting up.

"Central to EMS-5. 205 Fourth Avenue, apartment 3, second floor. Lift assist."

*I get in the unit and pick up the mic.*
"M5 responding code 1."
*I pull out of the complex and turn left onto Trulit Interpass. In a few minutes, I pull up in front of the apartment and head inside, followed by Chet. We are met by a middle-aged man, probably the patient's son. He leds us into the man's bedroom.

"Sir, ERT-EMS."

"Hello there, sonny. I fell out of bed and my son can't pick me up. Can you gents help me?"

"Of course, sir. Now, please just brace yourself."

*I place my hands under the man's armpits while Chet gets his legs, and we lift the man back into his bed.*

"There you go, sir. We'll be leaving now. If you have any trouble, please call us."

"Of course I will, sonny. Y'all are good men."

"Thank you, sir."

*We go back outside to the unit and climb in. I pick up the mic.*

"M5 en route back to complex."

*I drive back to the complex and we park.*
