Smoke Explosion SAR

*I grab the other end of the strecher and walk with Floan to the outside.*

'Well the smoke is getting lighter out here. That is a good thing.'

' yea no kidding '

* We continue to walk forward and I can see the glare of light, We keep walking towards the outside until we come to the door, Colwell and I finally end up outside and take the patient over to the RA, I look back to see that Jack and Tim have made it outside with their 5 patients and they continue to take them over to the green area. We get the patient to the RA and set him down outside on the stretcher that was set-up for us. I take my mask off and start to do a more thourough check on him with Cory. *

*I grab the neb treatment and set up a dual neb. I hand it to Floan.*
'Here Lt., start this while I listen to the lungs.'
*I grab a stethoscope and listen to the lungs. They a really whezzy. I take a full set of vitals: HR 130, B/P 134/96, RR 28, SpO2 90%. I turn the O2 up to 15 percent as we wheel the patient to the yellow area.*

* I Mix 2.5ml Saline with .5ml into the neg and administer it to the patient, he is still struggling to breathe and the treatment seems to be of little effect. *

' Cory we may need some Epi, either way he needs to be transported now. John can you assist the EMS units with the other patients please. '

*I head over to the RA*
'Hows your crew doing Lt.?'

'Got it Lt.'

*I take off my SCBA and coat and head over to Jack and Tim.*

'Ok guys what is the status of these guys?'

' Ok I guess, just really tired. There are still i think 4 more people left inside somewhere. '

* I watch as John heads over to the other other patients and helps treat them. *

* I walk over to floan*
' I still got 30 mins LT, i should be good to re enter if you need'
* I grab a water bottle and hydrate myself real quick*

' Not by yourself your not, Take Jack and John, go get the others inside. Im gonna stick with this guy until he gets transported, You take charge on the interior but send me sitreps. '

* I hand Tim the Floodlight and give him a nod. *

( Sorry for my absence u guys I passed my paramedic test so now im back. )

* I walk over to the LT and Hutch and see them frantically working on a patient. *

' Anything I can do to help? '

'alright lt'
* i walk over to jack and john*
' you guys ready, hydrated and enough air?'

' Jamie go and help with the patients over on the other tarp if u could please. '

" Pump-11 Command to OSC can we get another unit out here for additional manpower with triage and med assistance?. "

* I continue helping the patient with his breathing treatment. *

( Tim, godmod Jack. He said he was going to be gone for a few days, Colwell is good though. )

* jack gives me the ok and is ready*
' Colwell, you good?'
" 11 matthews to triage, can i get an accurate count of persons we have outside the structure?"

'Ya Tim, I'm ready to go.'

*I put my coat and SCBA back on and get ready.*

(Godmod me for a few hours. Mother's Day activities going on right now.)

"RA1 to P11 Matthews we have 10 outside the structure with 5 still missing, by COPA's original count. 2 Black, 2 Red, 1 Yellow, 5 Green."
'Jamie, you and Doug, both have 1 red for you to work on.'
*I turn to Lt. Floan.*
'Can you ask Jess to call for the MCI bus, for our Yellows and Greens, the cornorer for the 2 blacks, and 1 additional medic truck for coverage when the RA and M1 have to transport the reds.'

' got it Cory. '

" Pump-11 command to OSC We need the MCI bus, the ME and additional ambulances for medical coverage Over."

' alright guys lets go'
* I grab a halligan and walk up to the entrance and strap my mask on*
" Pump 11 interior to OSC, 3 pumper members entering the structure, be adivised smoke has been greatly reduced, it's clearing up"
* as we enter the building we can see much better and dont have to crawl anymore but can walk upright*
* we continue down the hallway and i notice the rooms we were already in but check again just in case*
* after searching everywhere i can imagine i forgot we didnt check the bathroom, we heck the womens and find nothing then we walk into the mens bathroom and find 2 males and 2 females with wet paper towels over their mouths and coughing*
" Pump 11 interior to floan and command, we have 4 patients inside a bathroom, all green tags, just need some o2 and fresh air, we'll be bringing them out the main entrance to triage" BREAK
" 11 interior to command again, uhh can you do a second 360 of the building and see if we have other exits anyone could of left in, uhh we've looked everywhere at least twice and havent found anything, still exiting out the front with 4"

* Jack, john, and i follow the hallway back out side and leave our victs with triage*
' alright guys, lets start popping some windows in there, dont break them unless you absolutely have to, lets check the locks on them first, lets get the rest of the smoke out of here, it's been a long day'

*I turn to follow Tim into the building again to do ventilation when a man stumbles out the door and collapses.

'We got one that just stumbled out side!'

*I quickly run over to where he is and pull him away from the door to get him out of the smoke cloud. The man is out cold. I listen for a breath but don't hear anything.

'Some get an AED i'm starting CPR!'

*I take my mask off and give two breaths to the man before starting 30 compressions. 1..2..3..4..5..6...

* i ditch my scba and jacket as i hear jack call out and i run to the pumper and open the cab and grab the aed and run it over to jack and kneel on the ground*
* i cut the mans shirt off and put the patches on him*

*I head over to Jack and Tim.*

'Jack, do you want me to take over CPR?'

*..25..26..27..28..29..30 I see Tim bringing the AED and putting it on the man. The AED gives the Shock advised call and charges up. Finaly it calls for us to clear.

'I'm Clear, You're clear, we're all clear'

Tim hits the button and the man jolts. I give two more breaths and begin compressions waitinf for the AED to do a final eval.

' john take over my spot, i got the patches on but thats it'
" Pump 11 interior to command and copa, all personell are outside, we have 1 male that has fallen, cpr in progress and aed being set up, we need a medic unit to the front door forthwith"
* i run over to medic 1 to grab their crew*

'Ok Tim.'

*I go over to where Tim is and I take over for him on the AED.*

'AED: Shock advised.'

'Ok Jack I am going to shock. Your clear, I'm clear, everyone clear.'

*I press the shock button.*

'AED: Shock delivered, Continue CPR.'

* I run over to the guys doing CPR. *

' Here I'll take over, JAMIE GET OVER HERE BRING THE DRUG KIT AND ALS BAG. '

* Jamie runs over to me, She starts an EKG and I open up the Drug box and grab a bag of Ringers. I start the Ringers on the Patient and have Anders hold the bag. I dig into the drug box and grab a Vile of Epinephrine, I push 1:10,000 solution through the his IV. the EKG shows nothing. *

' How many times have you shocked? twice? Charge the Defib to 300. '

* I watch as the machine reaches 300 *

' CLEAR. '

* I shock the patient and he converts to Normal rythem. *

' Ok stop CPR. Get a stretcher over here now and transport this patient. Also lets get a serious patient count. '

*I finish changing my bottle and get ready to go back into the building.*
"Ladder 1 Luecknebach to Command, are we still missing more victims."

* I run over to John and assist in the cenversion. *

" LT. Im gonna grab the Stretcher and transport in my unit. "

* I run back to the ambulance, grab the stretcher and head back to the LT and his men. *

' lueckenbach, dont go in alone, ill go if we have to, but we have all that we're mentioned in the dispatch, but we can still go and check if there are anymore'
" Pumper 11 interior to command, show FF matthews and L1 Lueckenbach going interior for a secondary search"
' alright jack, lets go to the front desk and see if there is any sort of log in book'

" Pump-11 Command to Pump-11 Interior, can you give me a status report on the inside? Also advise on how many other fans we need if any. "

* I help Jamie roll the patient into her ambulance, Doug hops in the front seat and i close the doors. *

* we enter the building and i find the main entrance with the log in paper*
' Man, this place still uses paper, no elctronic sign in's or anything??'
* I look and count the spaces*
" Yea, 11 interior to uhh command, and floan, only 15 people signed in for today and uhh, i believe we had 15 out in triage, also command, i didnt see outside, but you might want to start PD for the force entry we had" BREAK " 11 interior to floan, we should be set on fans, its just about self ventilated now, were coming out"
* We walk out of the building and drop my gear out by the truck*

" Copy Interior. BREAK Pump-11 Command to OSC, All occupants of the building accounted for, Will have accountability on Pump-11 crew shortly. BREAK, Pump-11 Command to all Pump-11 Personell please rally at the Engine. "

* I walk over and open the rear cab door and set on the step*
' here lt' i say exausted and sweaty

*I head back over to the pumper and start putting away all of my equipment.*

' Ok you guys, we did it, good job and i mean that to each and every one of you. Hell of a job in there, for now take your coats off grab a water and relax. '

* I take my SCBA and coat off and walk over to Forsyth. *

' Jess we are all good over here, my men are tired and need a break. '

*I leave as Floan takes over CPR and take of my SCBA putting it into its compartment on the truck. I take off my jacket and place it over the drivers seat putting my helmet on the dash. and have a seat.

"Anders to Lt. Floan, I am at the truck."

'Well what did you think of this call Tim?'

* i walk over to colwell and shake his hand*
' good job man, we worked quite well together in there'
* i grab a handful of water bottles and give one to colwell, jess, jack, and give one to floan and pat him on the back*
' good job lt.'

' haha jack, well it was intense, i thought we would never find everyone, i feel like going back and never hearing that annoying gong again'

'Thanks Tim. I think we will work well together. Thanks for the water.'

*I take a bottle of water from Tim and take a few drinks as I sit down on the bumper of the truck.*

*I take the water bottle from Tim.

'thanks, and yeah I know what you mean Tim!'

'Yeah good job John, it's nice to have you on the truck! I think we all work together well.'

'Thanks Jack.'

' Casey, Can you rollup the hose. the rest of you guys lets get over to Triage and help Cory with medical. '

* I roll the straps of my turnout pants off my shoulders and grab a few pairs of black Latex gloves and start walking over to Cory. *

' Hey Cory i got 4 men here who are DYING to help with medical work, how and where do u need us. '

*I follow behind Floan putting on a pair of medical gloves as I walk.*

* I also slip my sholder straps off and put my helemet on and grab a pair of medical gloves and follow the rest*

* i walk over to find what i need to do*

*I come out of the building and pull off my mask. I go over to the truck and put my pack back on the truck and take off my coat an place it on my seat. I grab a pair of gloves out of the back of the ladder and follow everyone else over to rehab.*

*i come walking out of the building and head over to Rehab, once i get there i dress down to just turn out pants and a tee shirt.*
'hey guys wanna check me out?'

'Alright LT, those who are EMTs can help with taking care of the 2 reds, I need to get back to triaging these patients, so If you wouldn't mind, grab the RA1 medic kit and take over treatment of the reds with your crew. Also can you send one of you guys oover to cover rehab?'
*I grab the smart triage kit and head back over to the tarps to do vitals.*

(guys not to be a bear or anything, but we are in MCI mode, any one not breathing and no pluse unless it is witnessed by you, we don't work. They are DOA at that point, black tag them and move on, unless you see them drop that is.)

*I roll up the hose*

'the hose is all set LT.


*I go and throw on a pair of med. gloves.*

(don't forget, Im an EMT. )

*I grab the tools, leaving the fans where they are to get the rest of the smoke out of the building, and take them to the truck. Then I climb into thruck and sit and drink a water.*

' Thx Casey, can you grab a set of gloves and come on over to assist with these patients, Anders can you go assist with Rehad while Tim, Myself and Colwell keep going here. '

" Pump-11 Command to Ladder 1 Beard. can you and Luckenbach come on over and help with triage? "

* I go back to working on the Reds with Cory. *

"Ladder 1 Lueckenbach copies, heading over to help with triage."
*I start heading over to RA1 to help with triage.*

* I go back over to my patient using the Neb. He doesnt look much better and I begin to worry further. *

' Hey cory he needs to go to ETC right now. His color is getting more pale. the Neb doesnt seem to be working much. '

" Pump-11 to Medic 1, are you 2 able to return to the scene after your transport? "

* i walk over to the triage area*
' Lt, want me to work with the greens, and start to organize them? by the way, have we started the triage bus for them?'
