Airplane Crash - Fire and Triage

"Thanks Mc Guire" I say I get out the ECG look at the man with chest pain and he loses consuisness. "V-fib!" I say I get out the defibrillator and shcok him. Okay back into sinus rythm.I start an IV and begin oxygen and patch up his wounds.

'OK, the tree fires are out now. Should I help with triage, Lt?'

*i notice that most of the fire around the plane is knocked down and its a bit smokey in the plane so i move down the plane attempting to bust out the windows for ventalation*

(Airplane windows are extremely hard to break. So.. how are you breaking them? lol)
"Status update from all units."

((a few good wacks with a hallogan. they are only plexiglass. if you dont think that'll work ill get the K-12))
"ladder 1 to command we have triage in side the plane and are atemting to ventalate as soon as i finish that EMS can come in and treat the victems"

*I go over the smoke near the engine and the smoke then goes away.*
'Lt., All fire's are out here.....you want me to help with taking the people out in the plane or stay down here and walk them over to the ambulances?'
*I place my crosslay down and wait for the people to be handed over to me. I \direct four more people who could walk to BLS-1 and wait for others.*

( off topic, but you know how they test the windows? they shoot chickens out of cannons at them, just thought that was a fun fact)
"bruce: do you want me to start breaking down our supply line?"

*I bring about 6 more people to BLS-1 and put them in the green mat (so far that's eleven more people)*
"Ladder 1, do you have any wounded who can't walk up there? Pass them off the wing to me if you can."
*I stand next to the wing of the plane and wait for more people to be passed to me. I look back and make sure my crosslay is still connected just in case I need it.*

"no matthews i need you in here to help me get these vics out"
*i pick up a lady and hand her to Hannahan*

'Ridgeback, if yo need to transport any of the reds you can start doing so.'

"Triage to FD, you can take whatever you need, you can take a BLS-1 stretcher, Reeves, backboards, you can use a stair chair if you want, take whatever you need."

*I continue tr

*i walk down the plane checking my victems again*
"i need two boards for these patients the rest are able to be walked with assitance"
((can i start IVs as an EMT. i know different states have different standards. just wandering what we are allowed to do here))

((IVs are Paramedics and higher ranks only))

((gotcha cause my state you can start IVs if you are an IV tech. wasnt sure on the ERT rules))
*I get one PT backboreded and me and Hannhan carry him over to triage. go back get the other backboarded and carry her over to triage.*
"alright that should be all of the Vics, ill do one more check of the plane to make sure"
*i climb back into the plane and check all the isles and find nothing.*
"Ladder 1 to command the plane is clear of victims"

((I follow the rules of EMTs can provide medical treatment and offer over the counter drugs such as tylonal etc, however not administer it. Paramedics may administer. The offering is covered under law, just like if you started having an allergy attack and needed an epi-pen, I can put it in your hand, and let it drop to your leg injecting you and be fine, but I cannot legally put the needle in you without being a paramedic or higher.))

"Understood L-1, requesting all available medics assist with triage, and non medics assist with transporting the victims unable to move, or clearing dangerous debris from the scene."

*I carry the injured lady to the yellow triage mat and set her down softly.*
'Our medical crews will take care of you now ok?'

"That's everyone?"
*I go back to the wing of the plane and wait for Bruce to appear again.*

'Hannahan we have all of the vics out im comming down.'
*I climb down and start clearing debreas away*

'Ok... let's see if we could help more.'
*I walk over to the triage area and start to reassure them that everything will be ok.*
'Hey Mcguire, Zack, you guys need help?'
"Lt., the plane is clear of victims."

I move on to the next patient and think to myself, how on the world are we going to treat everyone?
"Yes thanks Hannahan"

I walk over to triage and get triage tags. I walk over to some one.

'Hello Sir, I'm Firefighter EMT Jansen, are you hurt?'
'Hi, it feels like if my leg is broken'
'Let me take a look, does this hurt *I touch his leg*'
'OUCH DAMN THAT HURTS'
'Yeah, thats broken. I'm gonna give you a yellow tag, an EMT should help you soon. And keep your leg still'
'Thanks, I will'

TRIAGE TAG:
COLOR: YELLOW
SHORT DESC: Broken leg (Femur, Left)

*I put the tag around his neck*

I go to another person and make sure they're comfortable and see that half of the patients are ok and they're in either green or yellow.*
'You guys are good for now it looks like ridgeback. I can't do much because I'm not an EMT....I could help with any lifting though.'
*I get some backboards form the ambulance and go back to the triage area.*

'Yeah, here, let me help.'

*I grab and EMT bag and walk over to a victim.*

'Hey, I'm Chuck Davis. Are you OK?'

'NO, my arm really hurts right here.' He drags his finger across the middle of his left arm.

*I grab where his pointing and he said it hurts.*

'OK here, let me put a splint on that. It's probably broken. Here, wear this yellow tag for me and keep that arm stable.'

*I grab a yellow tag and put it around his neck. Then I walk over to the next victim.*

TRIAGE TAG: YELLOW, BROKEN HUMUROUS

*i walk over to the triage area to help*
'Hi sir, My Name is Bruce. What seams to be the problem today?'

'I think i broke my leg! it hurts like hell!'

'Ok sir im going to help you'
*I take his leg and splint it, and place a yellow tag on him*

*I set the backboard down and talk to a female who says she feels just fine, but her neck hurts.*

'Ok ma'am, I'm going to put this neckbrace on you to make sure that if you have an injury to your neck, it won't be further damaged. Alright ma'am, if anything happens, let me know or let the EMT's know also, Ok.'

*I tag the lady.*

TRIAGE TAG: GREEN, MINOR NECK PAIN

*I go over to the next person and ask him what's wrong.*

*I walk up on a vic who doesnt appear to be breathing. i check a pulse none. i put a moniter on him hes flat. i walk over to the ambulance and grab a sheet and drape it over him. then lay a Black tag on top of him*
'I got a priority 4 over here!'

"all units assist in triage as soon as all debris are cleared for safety precautions."
**Over the radio to dispatch**
"Requesting additional resources to Airplane Crash."

"Roger that"
*Grab my trauma and drug box and go over to assist Bruce.*

"What do you have?"

'hes flat sir, no pulse. hes not breathing. its up to you do you want to call him or work him'
*i reach down and pull the sheet back so Garcia can take a look at him and check him for a pulse again, no pulse.*

*I go to the next person, bend down, and start talking to him.*
'Hello sir, i'm John Hannahan and i'm here to help you.'
*I start doing an assessment on him and when I get to his leg, he says ouch.*
My leg is hurting.
I start a focused assessment on his leg and rub it downward until he says ouch again.*
'It hurts here?'
Yea, do have anything for it?
'Hold on.....'
*I get a splint for his leg and splint it.*
'I'm going to put this tag around your neck so the other EMT's know how your doing ok?'

TRIAGE TAG: YELLOW, DISLOCATED SHIN

*I walk over to another woman with my EMT bag.*

'Hello, I'm Chuck Davis. What's going on?'
'My leg just hurts a little, as I fell when I was getting out of the plane. It's not too bad though.'
'OK, where does it hurt?'
'Right here.' She points to the bottom of here leg, right above her ankle.
'OK, you might haev sprained it or something. Nothing too bad, but I'll have the paramedics check it out.'
*I place a green tag around her neck.*

TRIAGE TAG: GREEN, SLIGHT LEG PAIN

I go over to the next victim and begin treatment. "BP is low, Pulse Ox is low, and pulse is high. I think he's in shock." I say as I put a red tag on him. I must find the internal hemmorage before he flat lines.He already is unconsuis so I start an IV and put him on oxygen and an ECG.

"Lt. this is Anders ETA 5 minutes"

*I go over to another patient and I see him just laying there.*
'Hello, sir, sir! crap. I got one who isn't breathing here!'
*The man is pale but I check his airway, his pulse, and he has one but it's really slow. I take out my pocket face mask and put on new BSI. I put it on his mouth and provide 1 breath every 5 seconds for about 2 minutes. The guy starts breathing again and I check his pulse. His pulse is normal and his skin is returning to normal color.*
'Sir, are you alright?'
Yeah, did I stop breathing?
'Yes sir...'
Ok at least i'm all right.
'Im gonna put this rebreather mask on your face to help you breath better.'
Ok thanks.
*I put the rebreather mask on his face then tag him.*
TRAIGE TAG: YELLOW: BREATHING PROBLEMS

*i leave Garcia with the victem. And walk over to the last Patient in triage.*
'hello sir, my names Bruce and im with the Fire department. what seems to be the problem today?'
'i feel fine. im just having a little trouble breathing'
'ok sir, i believe you may be in shock. im going to out some oxgen on you to help with your trouble breathing'
*i place a NRB on him and place a yellow tag around his neck*

*I walk over to another person.*

'Hi, what seems to be the problem?'
'My neck really hurts because I was pushed when we were getting off the plane.'
'OK, here's a cool pack for now and I can see if the medics want you to wear a neck brace.'
*I place a green tag around his neck.*

TRIAGE TAG: GREEN, SLIGHT NECK PAIN

*i walk up to davis*
'I think were done triage. now how are we going to go about transporting?'

I arrive on scene and step out of my POV that is parked near the Ladder. I see that Triage is done and look for something that needs to be done as I get my turnouts on from the trunk of my Colorado.

*i walk over to anders*
'he anders i need you to start packing our hoses off the ladder'
*i walk over to the ladder climb up on the platform and put the ladder down. climb down raise the out riggers and put the steel plates away. then i walk up to the pump pannel. shut all the lines down and fill our water tank*

I walk control panel and disconnect the line and walk toward the nozzel running the hose over my sholder to drain the water. I get to the nozzel and twist it off and put by the control panel as I straighten the hose for folding it on the truck, I do this again and all the lines are taken care off except the supply line. "Hey Bruce, can you help me with the supply line and then folding lines?
(( we shouldn't need to transport if this is a drill, they would have been actors, unless we use real situation traing but that would be inhumane lol))

*i help anders walk the suply line out and place it back on Pumper 11*

(( does the Ladder have attack line connectors? may sound like a stupid question but our FD doesent have a ladder and I haven't been up close to one. We are trying to get funding and funrasiers to get one though.))

((from my understanding yes. witch makes our ladder not a true "ladder truck" its actually a quint. withch is a truck with a turn table ladder and carries an assortment of hoses and ladders. a "true" ladder truck carries no hoses or water. but has a turn table ladder and an assortment of tools needed for the job. hope this clears things up for you ))

"Alright lets fold the hose now" I say climbing up on top to lay in in the bed

'lets not pack pumper 11s hose yet cause they might still need the. i think we should wait till Garcia or someone from pumper 11 tells us what our plan is. lets check to make sure we have all of our tools on the ladder'
*i reach in the cab and grab the clip board with the check list on it.

(( I was talking about the Ladder's... but I looked back and we only used the ladder's cannon... oops lol))
"Yeah ur right we should wait."

*i walk around and check all the compartment and see that every thing is in its proper place. close all the doors and go sit on the bumper*

I take a seat next to Bruce and hand him a water from the cooler in one of the Ladder's compartments "Some drill I missed. huh"

*i take the water from Jack*
'thanks. yeah it was a great drill'

"Looks like it. When we finish we should bring water to the other guys"

'yeah go ahead and set water out for rehab'
i grab the other case outa the compartment. set it on the bumber and open it up*

'Remember guys, think about who our giving liquids to...it might cause them to vomit.'
*I help with setting water out and start going over inventory for the equipment on the Pumper.*
