'Ok, Moon lie him on the cot. Jess can you and Doug handle rehab.'
"RA1 to DCE1 we have one here that I'll triage for you then bring him over."
**Starting with patient**
*Moon sets down the patient. I estimate that he is a 25 y/o male, he is trying not to move the R leg. I notice that he has an open femur fracture on that side and on the other side I notice the he may have a compound fracture of the femur. I get a set of vitals Pluse is rapid and thready, B/P is 70/40, resp 30 shallow and labored.*
'Doug get in here now.'
*I get the EKG out and place him on it. SVT rate of 200. I quickly put the pads on his chest and select SYNC at 100 J. The patient cardioverts to Sinus tach at 148. I'm reaching for the O2 when I here him gasp. He's breathing at 4 per min. I reach over and grab the intubation kit. I open his mouth and stick the tube into the trachea. I hook the patient up to the BVM as I assist his breaths. Doug gets in and takes over.*
'Jess I know you're swamped but can you give me a back board and 2 Traction splints.'

*I grab him a gatorade and bring it over to him and give him a quick kiss*
'Stay safe out there Bruce. Might need you to take care of me after this '
*I then grab a backboard and 2 traction splints and bring them to Cory*

*I walk through more charred rooms and hallways and eventually hear the hissing of water. I continue and find some of L1's guys spraying some fire. I walk into the room.*
'It's about time I found some people. Here, let me give you guys a hand. I've got full air still.'
*I point my line at the flames and begin to spray them with a steady stream.*

"DCE1 Copies, I'll prepare my teams, advise of incoming state please"
*I check the floor to see if anyone is free..*

I direct him to the child, freeing two medics

* looking over at Bruce and jess i start to get that Gag reflex feeling *

' Eiwwww u 2 go, get a room lolol. Jess u gonna be ok? '

* I have my vitals taken and upon approval I choose the red gatorade and step down so the next member can be checked out. *

**I look over to the perimeter and notice a gathering presence of reporters.**

"COPA, the press is going to want a statement from us. Should I go over to address their concerns? We'll need a victim count and a status on the fires for whoever briefs the press."

"I'll announce a victim count later on, but right now, don't talk to em, it'll just mess things up"

"Understood. They've been good about staying away from the perimeter, so we'll leave them where they are."

"L1 HRES to DC-1 what do you need me to do?"

'Hahaha. Yeah I'll be fine. I'm stuck on rehab for the rest of the call but I should be able to get back into regular duty after this call.'

*Grab my axe and head back into the building and think to myself wow this is getting tiring.*
"Cressler to command heading back in offices 1-4 checked heading to office 5 and preparing to search."
*I make it up to the second floor and notice there starting to beat back the fire some, but not a ton.*

"Continue checking the building to clear it. If it is good then proceed with suppression of fire."
*I check my phone again, it's Gab*

'Good you're Okay, is everything alright?'
'Babe, you know I have the best men and women in the ccountry, probably the world, we're gonna do this fine, I'll be coming home to you and the kids tonight, I promise. I've gotta go now, I love you'

(I'm headed off to bed now myself, I'll be back in say 13-15 hours.. I've got two active orders that should put in another few baddies, so continue on with needed, I trust you guys.. I'll be off at a command post or something)

*I get the backboard and traction splints from Jess. I put both the patients legs in traction splints, after putting him on the backboard. I recheck vitals, and they're stabalizing now. I put in an IV 18 ga in the L AC. I give 4 of MS for the pain he must be in, and I sedate him with 60 mg of etomidate, and a maitaince drip of 2 mg/min to keep him down.*
"RA1 to DCE1, I'll be bringing you a red, patient is a 25 y/o male, c/o severe leg pain found under a desk by FF's. Upon exam, patient has possible bilateral femur fractures, and we supect shock. Most current vitals are HR 146, B/P 80/50, R 12 assisted, Monitor shows Sinus Tach. Upon arrival to our rig, patient was found in SVT rate of 200, cardioverted once successfully. Pt. is currently intubated and we are assisting his ventilations. We have him on 25 lpms O2 via BVM/ETT, 18 ga IV L AC, Etomidate 60 mg initatial dose, 2 mg/min maintainance dose. I have him on 2 traction splints and on a back board. I'm rolling him out of the RA and am on route to your location now ETA 3 min."

I wait next to the Ladder with the ALS gear, the oxygen and the backboards on the strecher.

*I hear mitch talking*
"Mitch I'm still alone in here searching the building has been deemed safe and I could use another medic in here."

*i walk away and change my cylinder out and head back inside. as i make my way up to where i stoped last i continue along and come to a widow and bust it out then as the smokes clearing the room i see what appears to be a partially collapest seiling and desk that had slid through and decapitated the person below*
"L1 HRES to EMS triage. ive got one more black tag. hes been decapitated"

*I grab myself a gatorade and take a sip and wait for someone to come for rehab*

"Cressler to Bruce what floor are you on."
*I ask making sure I don't waste time by checking anything he's already checked.*

"im finishing up floor 2"

* After my guys are done with Rehab we all head back into the building. *

' Ok guys pick up where u left off, Jack and Tim lets go to 3, Casey, you and Austin take 2, Radio me when u have completed ur secondary sweep and we'll all converge on 4. '

* I start walking up the stairs with Jack and Tim until we eventually get to the 3rd floor, I look down that the room that was fully involved and see that it has all but died off. *

' ' Ok guys start like any other fire lets check the doors and the walls, I have no idea what awaits us on 4 but if there is fire u can be damn sure its gonna spread everywhere. '

"Rodger which offices have you check? I got 1=4 and i'm getting ready for 5 or where you just checking restrooms"

"i got both restrooms and office #6"

"Ladder 1 command to DC 1 Fire, fire on first floor is out. Checking for hotspots."
*I stand up from kneeling down and I go out to grab the TIC but I remember that I have to go to rehab so I go over to Cory, Doug, and Jess.*
'Hey Guys, how ya been.'
*I say taking off my mask with a smile on my face.*
'Good to see you.'
*I then grab my mic and radio to HRES crew.*
"Ladder 1 command to HRES, give me an update."

"Rdoger"
*I line up on door 5 checking for signs of fire. Nothing. I take a swing with my trust worthy axe and knock the door in. As I get in the door I notice a big pile of junk on top of a man.*
'Sir can you hear me ERT:FD'

Man:'Yeah can you help met."

"Cressler I got one in office 5."

*I make my way over to the man.*

Man: 'I can't move anything I have a lot of pain in my back.'

'SH&^ okay I'm going to get you out.'

"Cressler to mitch I need you in office #5 I have a man pinned under multiple items. Probably broken back. Bring the board and some straps. BREAK RA-1 I'm going to have another transport for you"

(this is going to be the last office I search for tonight after he's out we'll worry about 7,8,9,10 tomorrow so hopefully some people can catch up.)

* I get a radio from Van Der Water that there is several rooms on fire on the 2nd floor, I checkthe 3rd and find no other rooms on fire however many are simply too demolished inside to make any sense of contents. *

' Tim, Jack you guys go to the 4th and check it out, im going to the 2nd to deal with these roomd. '

" Pump-11 Command to Ladder 1 Command can you send a crew to the 2nd floor to assist with multiple room fires? "

"Im assiting in searching the offices"

"RA1 HT to command, Back at RA1."

*I see Hannahan come over*
'Hop up Lt. I'll check ya out'

"Cressler to P11 command the offices i've previously searched are starting to become involved."
*I step out and look down the hall.*
"It appears offices 1-2 are fully involved and office 3 is starting to show signs."

"L-1 Command to Moon, how bad is he pinned? Can you lift the rubble with Mitch to assist? We can try to get some airbags ready for you." BREAK
"L-1 Command to P-11 command, I'll send in two." BREAK
"Chris, come off of the ladder and go in with Jack Leuckenbach. Second floor to back up P-11."
*I put my hand down feeling kinda weak but I stay strong and notice my tongue is getting sticky as in dry so that's not good.*
'You guys have anything to drink?'

"Cressler to L1 Command can you find me an extra FF or can bruce help me. I need one person to make sure the patient doesn't move causing more damage while we remove the debris."

*I grab him a water and a gatorade*

*i move up to the 3rd floor to search*

" P-11 Command Copies SV-1 BREAK Pump-11 to Incident command can we get another unit up for Supression. "

* I arrive on the 2nd floor and see Casey and Austin battling fire in the 1st office, I look down the hallway and see the 2nd 3rd and 4th offices are all starting to show signs of smoke. *

' Casey this isnt bad here dont take a defensive stance, get in the room and start aggressivly putting this out, there are many rooms here effected. '

" Pump-11 Command to Pump-11 Deck Gun can you focus your efforts on the exterior room showing smoke on the 2nd floor North side "

*I see floan walk past me on the 2nd floor*
'Hey Lt. can you hold this patient in C-spine while I try and push some of this junk off him.'

* I get up from helping on the hose and go over to Moon. *

' Sure. '

* I look at the patient who is lapsing in and out of consiousness *

"RA1 to Cressler, what have you got?"

*Lt. holds the patient while I give a good heave and get the debris off him.*
'Okay at least that eased up some of his pain.'

"Cressler to any available I need that board up here ASAP he's slipping in and out of consciousness"

"Cressler to RA1 1 patient sever back pain no feeling in extremities slipping in and out. No visible blood or wounds probably internal bleeding."

"RA1 to Cressler, I'm on my way with ALS gear and full c-spine gear."
'Doug hold down the fort.'
*I grab the Jump bag, a back board, and the collar bag. I get out of the truck and hurry to the 2nd floor.*

"Cressler copies."
* I see cory running up the stairs. *
'Whats up busy day today.'

'I haven't got any information from him I found him here and removed that debris from off him. Doesn't seem to be reacting to pain stimulus hasn't moved since I got here'

" Pump-11 to RA-1 can you bring a trauma bag as well "

' Damn i hope we get him before he rushes up here. '

'Floan sorry no truama bag, you caught me I was hitting the floor landing. Moon here can you get me a set of vitals, Floan keep c-spine, Ill get a c-collar 0n him and call M1 so they know I'll be coming.'
"RA1 to DCE1 and M1, I have a 30 ish male patient, found trapped under some debris, patient is coming in and out of consciousness. Performing assessment now, standby for further."

"Ladder 1 command to Cressler, you still need that extra firefighter?"
*I take the water and slowly drink it down and it refreshes me and I feel ready to go again.*
'Woah, that felt good. Thanks, wait... am I done?'

* While holding his neck in plae i lean over and look downt he hallway at my crew continuing to fight the fire, they have moved to room 3 now, having extinguished rooms 1 and 2 . *

'Just gonna check your vitals'
*I proceed to check his vitals and they seem normal*
'Your good to go Lt. Oh and I'll be okay too. I hope I'll be full status after this call'

*Im assisting with crowd control and i hear what sounds to be someone injured*
'That cant be good'
*I turn around and i see a middle agge man bleeding from the face and holding is left hand*
'What happened Sir?'
'I burnt my hand getting out of the building and a window shattered in my face'
'Can i see that hand?'
*He shows me his hand, Its burnt around the fingers and starting to blister*
"PC-8 To COPA I need an EMT To my position I have an injured male. Im about block up from the fire staging area. He is bleeding profusely from the face and has 2nd degree burns on his left hand"

"L1 command negative on that."
*I check the mans vitals.*

Pulse:40bpm
BP: 60/40
Oxy: 89
Breathing labored

'Cory we need to get him out of here he for sure has internal bleeding notify them we have a priority.'

*I get the c-collar on and start doing my assessment. I check the head and neck I see some trachea deviation to the left and the L Jugular is distended. I quickly listen to breath sound which are absent on the L side. I reach in to my kit and grab the needle decompression kit. I quickly insert the needle into the space above the 6th rib in the 5th intercostal space. Ok puemothroax is fixed, I look at the chest and see a zipper scar on the patient's chest.*
'Great he's got some kind of cardiac history, scar from open heart surgery.'
*I hook up the EKG, Sinus Brady at a rate of 40, ST elevation in II, III, aVF, V2,V3,V4,V5,and V6. Crap, AnterioLateroSpetoInferior Infarct. I insert an OPA and he takes it. I hand the BVM to Floan.*
'Floan, I need you to ventilate.'
