Airport Assistance Call

'Alright, well jsut observe, don't say anyhting, I don't wanna piss off the guy with giant scisors anymore ' I say with a chuckle
"EMS, update on traige?"

*After applying the 4x4 wound stops to bleed*

'Mr. Adams do you have any more pain?'

*To which he replies he doesn't*

*I go ahead and ger a set of vitals*

'BP: 130/80 P: 100 Resp: 20/Regular'

'Alright Mr. Adams i'm gonna go ahead and wrap your head and have you set right over there'

*I then wrap his head, and direct him over to a grass area and have him sit down, and make sure he has a green tag*

"Jansen to COPA, on my way to assist with triage over"
"Jansen to EMS Command, I'm heading to assist with triage, you copy?"

I jog over to the triage area

((Remember to update number of casualties and their triage status after your posts guys. And remember there is 10 passengers plus crew. One crew member is having a heart attack, so if we could get him extracted forthwith, that would be great.))

(Stop using OOC, use IC ways of saying the same thing!)
"COPA and EMS Copy Jansen" I say as I get a 10-4 from Riles over his EAT device, since he is still getting the basic triage underway. I just hope to god that the guys remember to update the EAT tags for the patient this time, not just the regular tags

*I turn on my scanner listening to PFD, PPD, and all other local agencies. I hear that PFD is working a head on MVA and that PPD is working a burg that just occured. I also check my EAT if any other calls are going on for us.*

I arrive at the triage.
'Hi Lafferty, where do you need me?'

'Jansen, you might want to check with Riles he should be around here somewhere'

'I'll radio him'

"Jansen to EMS Command Riles, where do you need me?"

"RA-1 to Jansen, can you do a quick rescue for us? We have a possible arrest in the cockpit, the copilot. ATC reported a possible AMI, and with how long its been..."
*I put the radio down, and pray to god that they get him out before he codes.*

*After Cory gets off the radio, I turn towards him*

'I can't totally speak for the rescue crew because I don't know the conditions inside the aircraft, but we've known about the co-pilot since the plane got down. He should have been out by now. I mean, other victims are coming out, we should have been transporting the co pilot by now, and it's as if he hasn't even been located yet.'

*I stand up, open the rear doors and step out. I fold my arms and lean against the open door and watch the fire crews operate around the aircraft. I glance over and see victims being treated at triage, then turn back towards the plane*

((Sorry, haven't been on all day.))
*I run into the cockpit of the plane, to which I see a man lying on the ground. I quickly pick him up and run outside towards the triage area. Then I set him down gently on the tarp.*
'Hey guys, I got the copilot, he's currently unconsious.'

*I see Chuck with the patient. I get out of the truck and have Doug help me grab the copilot and get him on out cot. I feel for a pluse...Nothing.*
'Doug start CPR.'
"RA-1 to COPA, we have an arrest."
*I grab the monitor and throw the 4 lead on and turn on the EKG. I see V-fib on the monitor. I get an 14 ga in the EJ and then get ready to intubate. Once I have my tools ready, Doug stops CPR for a moment to let me get the tube in. I have Doug give 2 breaths, and confirm tube placement. I the attach the Defib pads, charge to 200J.*
'I'm clear, your clear, ALL CLEAR!'
*I look up and down the patient, then hit the Shock button. I see the rythm change to Sinus brady rate of 30.*
'Doug check for a pluse and get me a B/P if there's a pluse.'

*I pick up the arm and check the pulse. I feel 12 beats in 15 seconds. 15x4=48*

'Got a pulse, 48bpm.'

*I attach the BP cuff to the EKG. I let it inflate, and it reads 60/45.*

'Let's get him in the truck. The saline and drugs are in there, helps for a quicker transport.'

*I drop the EKG on the head end of the stretcher, facing Cory so he can monitor it. I grip the bottom end bar of the stretcher tightly, as Cory gets the head end. As we prepare to lift I pull the black lever that unlocks the wheels and lets them drop*

'1...2...3...'

*The wheels drop, I gently let down the stretcher on it's wheels. We roll it down the taxiway back to our truck. I open the back doors, push the bumper up and in, then roll the stretcher into the ambulance, then pull it back out making sure the wheels lock. I hold up the stretcher as Cory brings the wheels up, then I push it in until it locks.*

*I close one door then get in through the other. I put the EKG on the counter top, turn on the oxygen, and attach a tube to the saline bag.*

(I'm putting anyone on probation that doesn't use the victim counter starting this post.)
"Copa Copies RA-1, advise status"

(I apoligize, I thought it was obvious that we had 1 pt, but 1 Victim Red tag, unknown on the others.)
*I turn to Kaizer.*
'I think I'll start a Dopamine drip to take care of that B/P unless you have something else you want to use. What do you think? 10 mcg/kg/min?'

((Sorry for the short post, just heading off to work))

'I figured to go for the mid range with how low that B/P is but you're the boss.'
*I grab a secondary set and the dopamine bag and hook them up.*
(mumbling)'Ok, about 220 lbs, so 20 drops for 5 mcg/kg/min, with 1600 mcg/ml consentration.'
*I set so that I have a drop every three seconds and what closely. A few minutes later I take a new B/P.*
'Doc, new B/P is 80/60, you want me to bump him up to 10?'

* I glance at my watch.
'Yeah, crank it up.' * I kneel and switch between leads on the ECG.
'I'm seeing T-segment elevation in I, aVL and V1-V5 with reciprocal changes in the inferior leads.' * I over at Hutchison. 'Anterior wall I'd say. Have a peek to confirm, will you?'

*I take a look at the EKG after cranking up the dopamine to 40 gtts/min.*
'I think we havve Septal, Lateral and Anterior involvment here, doc. Want me to set up for leads Vr4-Vr6 and V7-V9?'

* I glance at my watch again. 'Yeah, but let's be quick. We should get going soon, extrication took a good while, and they'll need to check for internal bleeding before they can do much.'

*I put the EKG leads for V1-V3 on the right side of the chest and put V4-V6 on the pt. back. I check the EKG strip, and I see that there is posterior wall involvement.*
'Doc, we have elevation in V7-V9. Posterior wall involvement. Dean, light it up, go Code 3 to the ETC.'

"RA-1 to COPA be advised we have ROSC and we're transporting 1 Code 3 to ETC."
*I feel the RA start moving and I hear Dean hit the siren as we get on the road.*

* I jump in the back for the ride to the hospital.
( 1 Red patient in transit. 0 remaining on scene.)

"Copy that, we'll have a trauma team waiting for a dump and go drop off"

*I look around, I then think to myself 'I don't seem like much help here'*

"S1C to COPA, I'll escort RA1 code 3."

*Jumps out quick, grabs my ALS bag and throws it in my passenger seat. Shuts my drivers door and throws on my siren and starts working my way with RA1 to the hospital*

*I notice the patient is starting to try to wake up.*
'Uh, doc...we got a situation, I think we need to push some versed, before he tries to pull out the ETT.'
(Sorry for the short post, on call here. Severe thunderstorms + copious amounts of water = Flash Flood warnings and a night at the fire house.)

*I walk back into the plane hoping the guy makes it. Then I come up to another person who's holding their head too.*
'Hello, what seems to be the problem?'
'Oh hey. I'm just a little shaken from the landing that's all, just a little headache.'
'Oh alright. Let's get you out of here and have the medics get you some asprin or something.'
'Ok. Thanks.'
*I walk out of the plane with the women behind me. We walk towards the triage area.*

VICTIMS: 3

*I see Davis leading a patient out of the airplane, I meet him as he reaches the triage area*

'Hey, Davis i'll take him if you need to get back to the plane'

*I help the man down to the ground*

'Hello Sir, i'm Dave, i'm with the ambulance service. How are you doing today?' (Other than the obvious)

*The man identifies himself as Mr. Langley*

'Alright Mr. Langley where are you hurting?'

*He states his head*

'Alright sir i'm gonna get a set of vital signs'

'BP: 120/80 P:100 Resp:20

*After getting a set of vitals I turn my attention back to the patient*

'Alright Sir, i'm gonna palpate your head if you feel any severe pain let me know'

*Upon palpation I note a good size bump on the frontal lobe region.*

'Do you have any neck or facial pain Sir?'

*He again states he does not*

'Alright sir, just sit tight here and we will get you transported to the hospital soon.'

*I then rip off a green tag, and put it around his wrist*

So far triaged we have:

Patient Number 1: Green tagged

Patient Number 2: Red tagged/enroute to hospital

Patient Number 3: Green tagged

* I watch the patients get tagged, and as I spot two greens just cleared by Lafferty, I call out to em*
'Hey guys, over here, follow me'
They walk over to the edge of the green mat
'What's wrong with you two?'
They tell me their injuries, and then I tell them they'll be fine, and they just need to wait here for a little while while a bus is en route to take them home.
'Once they have everyone off the plane, the Airport Authorty has a spot set up in the terminal where you guys can just rest'

"COPA EMS command, be advised, the mini bus is en route, ETA 10 minutes. Hold all patients until everyone has been traiged, then we'll transport all green patients.

When I pull up on the scene I grab the ALS bag and wait for more victims to be brought out. "ALS 1 on scene"

*I walk away after seeing the man get treated. I head back all the way to the plane. Then I step inside and notice a women holding her stomach. I go up to her.*
'Hello, I'm Chuck, are you alright?'
'Yeah, just had the air knocked out of me. My seatbelt was a little tight on my stomach when the plane landed.' *She takes in a big breath.*
'Ok, let's get you checked out. You'll be alright. Just follow me.'
*I walks with me to the triage area while holding her stomach and walking a little bent over. We finally reach the triage area.*
'Hey, here'a another. Probably green, just got the air knocked out of her.'

*I see Davis bringing a lady toward the triage area*

'Thanks Davis'

'Hi, my name is Dave and i'm with the ambulance service'

'What seems to be wrong?'

*The lady who identifies herself as Mr. Davis's wife, states she got the wind knocked out of her*

*I notice she seems to be really guarding her abdominal area*

'Alright ma'am i'm going to get a set of vitals'

'BP: 130/80 P: 98 Resp: 23

'Are you still having trouble breathing?'

*She states, she just can't catch her breath.*

'Alright, i'm going to get you started on some O2 that ought to help a little.'

*I set her up on 12 m/l via NRB*

'Now, Mrs Davis i'm going to palpate your abdomen and you let me know if you encounter any pain'

*As I palpate her abdomen I notice her jerk back a bit as I push on her stomach region*

*I then look an notice a contusion*

'Mrs. Davis everything looks ok to me, your seat belt was in working order for sure. You might want to go to the hospital just so they can check you out a litt more through than I can.'

*Mrs. Davis then states she would like to be checked out*

*I rip off another green tag and put it around her wrist*


Patient Number 4: Green Tagged

((That's my wife?! Huh?!))
*I go back to the plane to get someone else. As usual, I walk inside. There are a lot of people that look normal, so I cup my hands around my mouth and yell:*
'Attention! I'm going to take 3 people at a time to come off the plane. You three, *I point to 3 people* come with me first. I'll have you get checked out by the medics and then you'll wait for a bus to take you home. Comprendo? Ok, follow me.'
*I escort 3 people out of the plane to the triage area. I figure there are enough medics, so why not bring the people in groups to get things done a little quicker.*

VICTIMS: 7

(One of the patients on the airplane is her husband) (By the way, instead of saying victims you might say patient's)

*I see Davis on his way with three patients*

'Thank's again Davis'

*I walk up to the first patient and introduce myself*

*To which he states his name is Mr. Davis*

'Alright sir, i'm going to get your vitals'

'BP:130/80 P:110 Resp: 22'

*Mr Davis becomes worried that his vitals are a bit high*

'Sir, considering what your body just went through your fine'

'Do you have any pain or anything?'

*Patient states he is fine, and refuses further treatment*

*After explaining the consequences of not accepting treatment I have him sign the refusual form*

Patient Number 5: Signed Refusal of treatment

* I unzip my drug bag.
'I'm going with Propofol. Reaches optimal effect quicker, and has the same amnesic properties. Big guy this one, 100 kg?'

* I draw up 200mg of Propofol, and administer it.

* I look at my watch. 'How far do you reckon we are from the hospital?'

"The airport isn't too far from the complex. Probably no more than 2 minutes."

*I grab the chart and start filling out the information I can*

'Alright, good.' * I get my radio.
"EDRC to ETC."

"Go ahead EDRC."

"ETC, I'm with RA-1, we're bringing you a pilot, male, approximately 50 years old, suffered an MI during emergency landing. Went into v-fib, cardioverted, and returned to sinus. Currently stable, sedated and ventilated. Have a team stand by, he should go straight to radiology. And have the cath lab manned. Vitals on arrival, we're about two minutes out."

"Copy EDRC, ETC out."

*After taking care of Mrs. Davis, I walk up to another patient*

*And introduce myself once again, and he states his name is Mr. Clancy*

'Alright Mr. Clancy i'm going to get a set of vitals'

'BP: 110/90 P:90 Resp: 22'

'Sir do you have any kind of pain?'

*The patient states that he feels fine, and if he encounters any pain he will consult his personal physican.*

*So, I make sure he understands the risk he runs on not being checked out and have him sign a patient refusal form*

Patient Number 6 Signed Refusal of treatment

I go over to patient 7, "Sir how are you feeling"
"Just a head ache from hitting my head" I take a lood at his head and eyes and they are fine.
"Ok, I'm going to take a lood at how your vitals are ok."
"I understand"
BP 120/80 Pulse 110 "Probally high from anxiety" I say
Resperations 16 and Pulse ox 97%
"Alright sir after I put this green tag on you I want you to go to the green area ok" "Ok" He says. I put on the tag and he goes over to green.

*I run back into the plane to get 3 more people. Again, I enter and point to the first 3 and ask them to come with me. Then I lead them from the plane and towards the triage area.*

Patients found on plane: 10

*Once Davis brings the patients over I get on a new pair of gloves.*

*After introducing myself, the patient identifies himself as Mr. Wilson*

* I get a set of vitals*

'BP: 120/80 P:90 Resp: 18'

'Where do you hurt?'

*Patient complains of forearm pain*

*Upon examining his forearm I see a fractured ulna*

'Alright sir, i'm going to splint your arm and you will be good to go to the hospital'

*After splinting his arm and assuring distal pulses are still present*

*I then rip off a green tag and direct the patient to the low priority area*

*I then walk over to the next patient, who identifies herself as Mary*

*Mary declines any medical treatment, and won't allow us to even get baseline vitals*

*So I explain and have her sign a refusual*

'Davis to EMS, I just have 2 more left on the plane.'
*I head back to the plane from the triage area while Lafferty treats the patients. Then I enter the plane and ask the two last people to come with me to the triage area. They follow behind me toward the medics. I finally arrive at the triage area.*
'Here's the last of them guys.'

"RA-1 to COPA we're out at ETC."
*I radio in as we pull in to the ETC. I slide down to the end of the bench and open the back doors of the RA to see the trauma team waiting for us.*

*I check on the first patient*

*Patient's name is Brad, complaining of a head injury*

*I check his vitals*

'BP: 120/80 P: 100 Resp: 18'

*I palpate the patients head and feel a good size knot on the back of his head*

'Sir, I don't note anything extreme, but you should get checked out at the hospital'

*He agrees and I place a green tag on his wrist and direct him to the other patients.*

*I then walk over to the other patient*

*Patient is Carl, and is Brad's 16 year old son*

*I check vitals*

'BP: 110/80 P: 90 Resp: 20'

*Carl complains of severe shoulder pain*

'Carl, it appears you dislocated your shoulder.'

'I'm going to sling and swathe your shoulder and we will transport you to the hospital for further treatment'

*I then sling and swathe Carl's arm, and place a green tag on his wrist*

''BLS-1 to ES-1 triage is complete ready to begin transport of patients to hospital.''
"Can you advise ETA of the transport vehicle"

((Am I even needed here or can I go back to the station?))

(McEvoy, all we are waiting for is the transport bus to arrive to take all patients that are going to the Hospital, so you are free, unless Riles or somebody needs you.)

(you should be able to God mod the bus coming on scene it's be over 12hrs so by burkes standards you can God-Mod it...and Chief you werent dispatched for it you just stopped by plus your the chief i'm pretty sure you can clear yourself)

"BLS-1 to dispatch: Transport Bus has arrived on scene"

*Once all patients are loaded up I walk up to Riles*

'Hey boss, i'll follow the bus to the hospital in Basic 1'

'And, i'll get started on all of those PCR's haha'

(Bus should be there, McEvoy can show up and leave whenever he deems neccessary, he would be nice to be there, but doesn't need to hang around after if he gets tired of being there, since Raven was appointed OSC)
"Copy confirmation bus arrived, please advise when prepared to transport patients. PAAP will be providing an escort to the bus, as well as a basically trained EMT to ride along"
I check the scene radar after checking a few things not call related, and then see that the cameras show most victims out.
"Any medically trained persons please assist with triage while non medicals continue extraction. Riles will coordinate"
