High level platform rescue

' Ladder 1 Floan to IC We have 4 victims, 1 Critical already Rescued, 2 non Critical 1 of which we are preparing to move now and 1 Unassessed at this moment, fire still out of Control and spreading. BREAK: Ladder 1 Floan to RA-1, Bruce and I are heading down with Burn victim now, prepare to receive. '
* Bruce and I begin the decent down to the Medical crew. *

"Units evac those patients as fast as possible and then I want units to attack the fire before it clams my FF's."

*While they're bringing down the patients, I grab a high rise pack and hook it to the 200 foot crosslay and charge it. I watch it blow up and pick it up by the nozzle.*
'Chris, put your mask on and hold onto my line. We're going up.'
"Ladder 1 command and Chris Tufts enetering the building to supress fire."
*I start my way up as Bruce passes me and I work my way to the top. I the victims and try pushing them by putting my line down and putting them near the window with the ladder sticking through hoping Bruce would see them.*
"Bruce, we put the vics closer to the window so they're easily accesible now. Get them out quickly. BREAK
"Ladder 1 to S1C, we're working on extinguishing the fire as quickly as posible. We'll keep in constant contact with you."
*I then pick up my line again and go through the room looking for fire. I see none until I come up on a hallway that's very dark and its not helping since its nightime outside so not much light could be coming through even if we had the windows open. I find two barrels in the corner of one room after feeling the cool wall to find it. It looks as if they're propped up like they're to be used for cover against enemies. I spray it with the water from my line and I figure since they're still upright, they still have some sand or something in it. I spray side to side determined to put out the fire.*
"Ladder 1 command to crew, can you ask the non critical vics what they remember from the blast? Anything on what the room looked like before the blast? S1C, we have extinguished two barrels in one room. Moving on to the next one now."

((on call it be command or IC if I have assumed onsence command.))

"Copy that ladder 1."

"IC to COPA, the fire is being taken under control by ladder 1."

* As bruce and I reach the bottom we hand off the Victim to the RA and grab another Stokes and head back up. *
' Ladder 1 Floan to IC the count now stands at 2 Victims rescued.
1 Critical Unresponsive = Rescued w/ Medics now
2. Non Critical burn = Rescued w/ Medics
3. Broken limb victim = Still inside4. Unassessed Victim = Still inside Extraction team heading back inside now to continue rescue. Any chance of another unit assisting us with rescue efforts? '
* ALS bag on shoulder and toteing a stokes Bruce and I begin our ascent back up the ladder to continue the rescue. *

"S1C Update please?" COPA Out.

"IC to COPA, We are currently extinguishing the fire, we have two patients rescued, we still have 3 needing to be rescued. The injures are Critical, Semi Critical, 2 non-life threatening, and one unknown. The fire was started by an accidental flare shot and injuries were due to that. Ladder 1 is going to extinguish the fire with Hannahan and Tufts, Floan, Beard, and Jess are going to be rescuing the rest of the patients and evaluating them."

*goes back to typing in on the computer having my binoculars, and listening to radio traffic*

*We get the patient from Ladder 1 and load him up in to RA-1. I turn to Collins.*
'Get me a set of vitals, Dean. Doug I need him on the EKG and I need a line.'
*I watch as Doug set up a 12 lead and then sticks a 14 ga in the AC. Dean reports that vitals are pulse is 150 and weak at the carotid, B/P is 80/50, respirations are 26 shallow and labored, lungs are clear.*
'Dean get him on a BVM, 25 lpm of O2. Doug I want you to bolus 500ml of NS.'
*I check the monitor and am not suprised to see SVT. I reach over to the EKG and get out the pads. I place the pads on the pt. chest. I set the EKG to Sync cardioversion. I look up and down the pt.*
'SVT we need to cardiovert. I'm clear, your clear, all clear.'
*I press the charge button. After confirming everyone is off the pt., I press and hold the shock button till a shock is delivered. I see the rate go down a bit but stays tachycardic.*
'Dean get us going, we need to go Code 3.'

"Rodger that S1C" I input the info from McEvoy into the COPA Program, now I know I have the latest scene info. "COPA to S1C please keep updates flowing into COPA." COPA Out

((Floan we now have 4 PTs and you missed the PT with trouble breething))
*i move over to asses the PT that i just pulled out of the fire.*

*I look up through my mask and complain asking myself why they didn't they turn the lights on. I slam my hand down and keep going with Tufts right on my heel. I turn on my flashlight on my jacket but it doesn't help much. I try my best to follow the orange glow of the fire with my nozzle occasionally running into dead ends having to back up the hose which is a real nuisance. I then go into a room where the heat starts to building and the smoke gets thicker. I don't know what it was but something in my gut tells me to spray and just then, I see a bright orange flame amidst the smoke. It's the rear room so I'm hoping this is where the fire is even though I have no idea where I'm at. I move a bit closer and feel my hand touching something metal. I feel my gloved hand get hot and push the metal thing back seeing that it was the flare gun.*
'Yes! Finally.'
"Ladder 1 to IC, I think we found the fire.....standby."
*I spray into the smoke and more smoke rolls out as the glimmer get bigger them smaller. I see a shutter to the right of me and ask Tufts to open it. He does and gets back on the lines so the smoke starts to quickly rumble out and I start seeing a pretty big fire in front of me. I get a big smile on my face and brace myself for the high-pressured water. 'I'm goin' home.' I think to myself joyfully and I grab the handle and start to spray the fire. I sweep side to side in hopes that it will go out quickly but to no avail. 'I'm going to be here for a while' I say feeling some hot air on the right side of my face from the heat but cool air on my left from the open shutter. I finally get that fire out but realize that the most smoke is coming from behind a door in the back of the hallway. 'Remember my training....' I back up out of the room very slowly and start to make my way towards the back.*

*I set my jump bag down and again make my way back to BLS-1
*I grab a splint and set it on the top of the reeves and gently pull it out of the rear of the ambulance.
*I slowly push the reeves back to where Dave is and wait for the patient to arrive.
*I begin humming waiting for the patient to be brought to us

*I hear Masto start to hum, and I join in*

'Do you have a problem if I ride in the back with the patient(s)?'

'Go ahead captain.'
*I chuckle
*I then continue the hum.

'Alright sounds good'

*I crawl my way to the back room feeling the walls through the smoke that gradually gets thivker and thicker until I find the door. I push it open and a wave of heat blasts towards me and smoke plumes out from the doorway. I fall back with my line still in my hand.*
'Woah.. glad that wasn't what I thought it would be.'
*I get up and see my neon tape starting to look dark. 'Dang, I wanted this to be clean!' I think to myself in a somewhat disspaointed mindset. I grab my radio in hopes that Tim will hear me so I could tell him what to do.*
"Ladder 1 to firefighter Tim Mathews, bring up a TIC and follow the line inside to me K?"
*I get up from falling back due to the smoke and air pressure sending me back. I then make sure that I have Tufts with me.*
'John, you alright?'

"IC to COPA, L1 has located the second fire and is going to start extinguishing"

*Sitting in drivers seat of my truck, being puzzled*

' Ladder 1 Floan to all units on scene, we need to get a handle on the total patient count then can i get a Total victim count from all units to make sure we are not missing anybody on our assessment. '

* I make my way with bruce over to the patient that was brought out of the fire and begin a primary and secondary survey. *

"IC to L1, we have a total count of five patients, the first rescued had difficulty breathing and was evaluated and declined medical, the second was the unresponsive patient RA-1 has, the non critical burns is taken by PEMS, BLS-1 will transport the broken leg victim, and PEMS will handle the unknown victim too."

"IC to COPA, you also copy my last traffic?"

*waits for response*

"IC to COPA, we have RA-1 for ALS medical onscene, BLS-1 will handle non critical, and we have one PEMS ALS unit onscene that will wait for the unknown patient and will also transport the non life threatening burn victim."

Current victim count: 3 rescued out of 5, one still needing to be rescued, and one still MIA.

"Understood command, please also make sure to update victim statuses in the EAT"(the little victimcount at the bottom of ypour posts)

'alright john our primary is to get him outa here then we can treat him when we get down and the others can take care of the fire'
*i get the stokes baskit ready*

"PEMS will transport both burn victims to the hospital."

*I get up and see the fire almost very clearly and grab my nozzle and start spraying. It hisses with a vengeance back at me then gets a little bigger then get smaller. I then move closer to the fire while still on my knees and I see I have about 15 minutes left on my SCBA out of the corner of my eye. After about 5 minutes, I stop since the fire is finally extinguished. I grab my radio and radio to COPA and S1C.*
"Ladder 1 to COPA and S1C, the fire is now extinguished in the building. Do you copy?"
*I then start to back up and peek through other rooms to see any signs of fire but the smoke starts to clear up enough that I could stand up. I then get back to my window and see the that the soldier is now gone. 'Good.' I say to myself and I bump up on the window almost tripping on a wood block.*
'Ah!'
*I almost hit my stomach against the ladder but feel fine after getting up. I put my leg over the ladder rung and grab onto the rail. Chris gets on it too and we start our way down as I have my hose in my hand. The hose starts to form somewhat of an oval shape as we descend down the ladder and I allow the hose to slam on the floor with a thud but I still manage to hold the nozzle. I put the nozzle down and decrease pressure to the crosslay. I then take off my SCBA and change it out but ulitmately take it off of my back because I feel not much need to go back inside. The smoke dies down a few minutes after I come out but I watch the building anyway just in case.*

( wow the call went fast, did you still want the tic? cuz your outside now..or am i running it up and checking for extn.?)

* I count to three and Bruce and I lift the patient and start bringing him down the ladder to the awaiting medics *
' Wow that count for me coudltn have gotten any more screwed up. '

'Tim, bring the TIC up. I'll be behind you with a line.'
*I grab my line again and put on my SCBA getting ready for the sort of long trip up the ladder again. I get myself together hoping visibility has increased a lot more than before . I sling the line over my shoulder, get onto the rear ladder of the truck and onto the main ladder and start to work my way up.*

"Copy that ladder 1. BREAK. Medic units are clear the scene when ready to transport. The hospital is ready for the 4 patients we are bringing in."

"IC to COPA, PEMS is clearing the scene enroute to the hospital code 3 with a semi critical burn victim and and non life threatening burn victim."

*writing info on my notepad in my truck and on the EAT*

"SV-2 to L-1, let me know when I can discoonnect the line"
*I get the hydrant wrench ready*

"10-4 SV-2."
*I put my radio back on my shoulder then continue back into the building...this time standing up. I look around but see nothing but, determined to find something, keep looking around.*
'You see anything Tim?'

((I'm assuming BLS-1 has recieved our patient from F.D. at this point))

*After making sure the splint has been secured to the patient, we make sure the patient is secured to the cot, and want to start the patient on O2 and check vitals*

'Hey Masto, can you go ahead and start the patient on 5 liters N/C'

'I'll get vitals'

'BP 110/70 Pulse is 90, Respirations is 20'

*I then push the cot into the truck and make sure it is locked in place*

'Masto we are ready to go when you are'

'Basic 1 to dispatch: Be advised we are enroute ERT-ETC non-emergent'

*As we are leaving the scene I contact the ER and advise them with what we have. After an uneventful trip to the hospital, I notice we are backing into the ER, I then get the patient ready to transfer care*

'Basic 1 to dispatch: Be advised we have arrived at our destination ERT-ETC'

*We then push the patient into the ER, where a nurse walks up to take charge of the patient, and I advise her of Vitals and interventions conducted.*

*After patient care was transfered to hospital staff, I finished our PCR and made copies and turned it in. And, then advise dispatch that we are back available*

'Basic 1 to dispatch: Be advised we are back available.

* Once we are down and patient is handed off to BLS-1 Bruce and I take a second then collectivly go back up the ladder for the last grab. *

' Ladder 1 Floan to IC, Bruce and I are heading back in for the last victim, he has 3rd degree burns on 1 leg. '

'The last person is with RA-1 John. Help me with overhaul. Start putting things away. Bruce, help him.'
*I go through the rooms and see nothing*
'Anything yet Tim?'

*I pick up a relatively large oxygen tank and bleed it quickly
*I place the regulator onto the tank
*I open the tank with the nonferrous wrench
*I turn it back one round, and then check the psi, finding it is 1600 psi
*I connect the mask to the tank
*I place the mask on the patient
'We're a go'

' Copy that LT understand last patient is out and all are accounted for I will join you for Overhaul as soon as I can reach you. '
* I begin slowing things down now that I know all victims are out, I start down the ladder and make my way over to the truck to grab a Pike. *

"IC copies your traffic L1, PEMS is standing by waiting for your patient next to my vehicle."

*looks over at PEMS, gets there attention, and tells them their patient is coming down*

"Ladder 1 to IC, 10-4. Tell them to come over near the ladder."

"That's what they were going to do L1"

* I walk over to the truck and take my turnout coat off, sit on the bumper and bury y head in my hands thouroughly frusterated. As i sit there I begin to think to myself ( Never before have I been so completly frusterated and confused as to what just transpired ). * I look up to see Hannahan waving me over to help in the Overhaul, I grab my Pike and head into the building to assist. *

" Lt i got one small hot spot in this wall" * i yell*
* John walks over to me with the line and Floan with the pike pole*
* I show them the small spot through the tic and they nod their heads*
" Alright John and John" * chuckle* " Lets take this small part down, no need for the whole wall to come down"
* I back up to let them get to work*

'Dean hand off your BVM to Doug. Doug I want to intubate to protect the airway, so go ahead and check the gag reflex, then prep for intubation.'
*Doug tell's me that the pt. does not have a ag reflex, and I set up for intubation. When I'm ready Doug moves the BVM away from the pt. face, so that I can tube them. I put my laryngoscope in to the pt. mouth and sweep the tounge to the left. I see the vocal cords and I pass the tube through. Doug attaches the ETT to the BVM and I heck placement. I confirm proper placement and secure the tube. I feel the RA move foreward and hear Dean hit the siren.*
"RA-1 to COPA and IC, we're en route to ETC, Code 3."

* I back up and lean forward aggressivly sticking the pike into the wall and pull out a chunk, it drops to the ground and sure enough a small flicker of fire shows itself, I stomp on it and put it out and keep going down the wall and ceiling poking in and seeing what pokes out *

'Hey, hey, you alright? Your not looking too good. I seen you about to fall apart out there...what's wrong?'
*I put my nozzle at my side and pat John's back and get in front of him and look him in the eyes.*
'Hey, what's wrong?'

* I look up *
' its nothing really but I'll tell u later, just lets get this done and get back. *
* I continue working the ceiling and walls. *

'Alright. Tim, you find anything else?'
*I start moving around to the rooms I went through and check kicking over some things but still find nothing.*

"RA-1 to COPA, RA-1 is clear of the ETC. BREAK RA-1 to IC, requesting permission to clear this call and assist on the Airport incident, if you don't need us for any more transports."

* Looking at Hannahan having alot of fun kicking things over i chuckle a bit *
' Ok John are we done in here now? ready to head back to the truck and pack up? if not there is a pile over there u can cannonball into lol. '
* I take my cover off and start heading back to the truck to put the gear away. *

* I nod my head to my shoulder and listen to the dispatch of the airport incident and look around seeing everyone doing the same*
" Damn, sounds like it could turn into a good one, No Lt. theres nothing else, lets get started packing up incase something happens at the airport and they need us"

"The units dispatched to the ARFF call should be fine, they were dispatched for a purposely for this call. Also S1C and other on-scene units notify COPA before leaving" COPA Out
