Two Car MVA

* I Pull the backboard up on to the road and hold it making sure it wont move*

*I let the rope down slowly so it will touch the concrete softly. As the cold air hits my skin, I let go of the rope to let Tim handle the patient easier.
"Cory, hook yourself up and I'll drag you up. Work on this patient. Jess, unless Lt, Raven needs you, come up after Cory. Raven, you need Jess down there?"
*I get my hands ready by taking off my gloves, rubbing my hands together, and putting the gloves back on. I then walk over to the harness rope and grab it in one hand waiting for Cory to tug on it so I could hoist him up. I then remember to update COPA so I grab my radio and press the PTT button.*
"Ladder 1 to COPA, the patient has been extricated and is on the highway now. ALS care is being provided."

*I tug on the rope and wait for Lt. Hannahan to pull me up.*

*I grab the rope and slowly tug Cory up. I then see the victim from the corner and think about life and death matters. I then use my adrenaline strength to pull up Cory quicker.*
'Alright, he's all yours Cory.'
*I walk over to the edge and see Bruce.
"Bruce, get up here and help Cory. Hook yopurself to the rope and tug."
*I once again get my hands ready to pull up Bruce and the rope.*

*I see them starting to pull him up. I run back to BLS-1 to get the stretcher. I take it out, and roll it over near where teh person is being brought up*

"Cory, I got a stretcher here. You want to transport with BLS and you ride with us, or transport with ALS?"

*i click into the rope and tug*
"Hannahan pull me up"

*I pull the rope once again using my strength and bring over Bruce. I gently let him down and walk over to him.*
'Alright Bruce, I want you to help Cory and Mcguire. Ok?'
"Jess, hook the line to the cutters for me and don't forget the fire extinguisher. Raven, do you still need that line? (2 1/2)"

'Lt. Hannahan, we're gonna transport in BLS 1 and I'll ride in with my medic kit, this guy doesn't have time to spare.'
*I pick you my ALS equipment and hop in to BLS-1*
'Mcguire, I'll need you in the back. Get me a set of vitals ASAP. Masto you drive, we're going Code 3 to ERT ED.'

(fire exstinguisher was already lifted up remember)
*I hook the line to the cutters*
'Lines hooked Hannahan'

(it was? ok)
*I grab the rope and tug the line up quickly then reach for the cutters, grab it by the handle and place it down near the Ladder.*
"Are you coming up Jess?"

*Botfish helps me take the victim out of the car. We lay him on a backboard.*
'Raven, the vic is extracted, anything else you need me to do?'

'I'll come up last after we get the equipment up'

*I turn off the generator, unplug everything, and put them back in the squad.*
'This vic is DOA, just wanted to get him out of the car. I'm free if you need me.'

*While waiting for Mcguire to get me a a set of vitals, I get 2 IVs set up, I open the airway and feel that he's breathing, but just barely. I get ready to intube, if things go south. I check the patients head and find a blood gash on the fore head, which I bandage up. I check the chest and listen to breath sounds, which are clear. I feel is abdomin which is very rigid and distended. Pelvis is fine, but patient is totally unresponsive. I sink 2 14 ga IV in to his ACs and for the NS wide open.*
'I could really use some vitals McGuire.'

*I wait for the line to come back down to me*

*I lower the line after unhooking the spreaders. I then wait for the tug or shout from Jessica. I start to sweat but feel good doing it since my body becomes warm but cools down due to the sweating.*
"You ready Jess?"

'Hutchinson, need some help? I'm an EMT.'

'Right now, I need a set of vitals, and Mcguire to get in so we can get off scene. If you can get me a set of vitals though that would be great.'
*Still waiting on vitals, I grab the monitor and set up for pluse ox, 12 lead, and CO levels.*

*I hook up to the line and give it a tug*
'Ready'

'Alrighty.'
*I pull with a mass amount of strength and pull Jess up kinda quickly. I never knew I had that strenght then out of breath, I go to the bumper of the ladder and sit down really quickly. I stand back up and watch Cory for minute help the patient, I get knida bored watching so I turn around.*
"Raven, how you doing with that stiff? You wanna hoist it up? (Do we call coroners or can BLS-1 take it?)"
"Ladder 1 to COPA, can we get some coroners out here?"
*I put my radio back on my shoulder then shout.
'Alright guys, lets pack it up! Overhaul time.'

'Is that all?'
*I sit in the Passenger seat and wait*

(BLS-1 and I are transporting our critical, since they are right here. Look a few posts above.)
*I hook the pt. up to the monitor and see he's in sinus tachy rate of 140, O2 sat. 93 and falling, CO is negligable. I put the patient on a 25 lpm via bvm and hand it off to Mcguire. I then give adenocard at 6 mg. to slow down the heart. I take a B/P it is 90/60, Resp. are assisted at 12 per minute.*
'The patient is going in to shock.'
*I cover the patient with blanket and raise the back board to a Semifowler's. I hang 2 new bags as my last bags are drained and switch 1 of the sets to blood tubing.*
'Masto, lets go hes going to crash if we don't get him to an OR soon.'
"Hutchison to Lt. Hannahan, please notify COPA that BLS-1 is now ALS and is transporting a Level 1 trauma to ERT ED."
(Report to ED in the Emergency Treatment Center.)

"Alright Hutchison. Ladder 1 to COPA, BLS-1 is now transporting ALS, Level 1 trauma to ERT ED."
*I put down my radio, go and grab my clipboard, and start checking over things. I realize something is missing and look back in the compartment where its supposed to be.*
'Where is the fire extinguisher?'

*I hop into the ambulance and sit in the seat by the head of the patient and assist in taking vitals*

'Let's go Masto'

*As we begin to pull away I walk to the cab and retrieve the CAD and put it on my lap. I put the vitals into the computer as Cory calls them out. When vitals and additional info are complete, I send it to the hospital*

*i walk over to the ladder unhook my harness and put it away*

*I put away the cutter and look around and remember that Ravens still down there.*
"Ladder 1 to COPA, request any available tow. One with a long winch."
"Raven, you coming up yet? Coroners are here. Strap the guy to the harness and I'll pull him up."
*The coroners pull up behind the Ladder and they get out with their stretcher and black bag.*
Hey.
'Hey. This guy died from the car accident. He was DOA when we found him.'
Ok. get him up here and we'll get him outta your hands.
'Alright just a minute.'
*Raven puts the guy in a harness and tugs on the rope. I pull on it very hard seeing that its heavy deadweight. I bring him up then the coroners put him on the road, bag him, put him on the stretcher and leave without even sayong goodbye.*
'Goodby to you then.' I mutter.
"Raven, you ready?"

'Well they have nice manners...'
*I walk over to Hannahan*
'I'll help you pull him up'

'Ok, hold on. I think the driver has to give Ewan the line or whatever since Ewans already down there.'
*The tow truck driver hands Ewan the line and Ewan attaches the line to the car. The car is then hoisted up onto the road, put on the truck and the tow truck leaves. A few minutes later, two more tow trucks pull up and take away the other two cars. Soon after, Ewan tugs on the harness rope and I pull him up. I then detach the harness rope and put it away closing the compartment after me.*

*I toss my ALS kit in to the back of BLS 1 and hand the two coffees up to Masto and Mcguire. I climb in and buckle up, grabing my mic.*
"Hutchison to Ladder 1, BLS 1 will be returning to scene, back in service AOR."

"10-4 Hutchison. You might want to just return to the station. We'll be done here in a few K? Ladder 1 to COPA, Tow has picked up the remaining cars and the coroners have picked up the dead body. Can we clear?"
'Alright guys, we can go.'
*I put down my radio. and drain the hose after disconnecting it then put it back after packing it. I wearily go back to my seat, but keep the door open just in case something happens. I turn up the radio and wait for something to happen.*

* i climb in the drivers seat and grab my head set.*
'ready Hannahan?'

"Hutchison to Ladder 1, I copy, I'll meet you back in quarters."
*I grab the ALS kit, thank BLS-1's crew for the ride and walk over to Station 1.*
"Hutchison to Ladder 1, I'm at quarters awaiting your arrival."

*I hop in the ladder taking off my helmet and sit down*

"10-4 Hutchison. Ladder 1 copy's."
'Sometimes, it could get like this haha. They're realy busy ya know... just gotta wait. How did you guys like that call?'
*I say through my headset with a smile.*

'Oh, I almost forgot'
*I jump out and put away my harness and helmet then jump in again*

'i liked it. Didnt get here till half way through tho'

'Thats ok. at least you did something. Dang, I'm hungry again.' I mutter.
*I check my texts on my phone to see if I have any new ones.*

'I liked the call too, shame about the DOA though'

* I get back in the truck, take my helmet off and lay it on the ground between my feet and put my headset on getting ready to be cleared*

'Yeah...I jumped a bit when I hoisted it up...almost dropped it haha.' I smile a bit.
*I close my door and put my seatbelt on. Then check to see if the emergency lights are on and they aren't so I just relax myself as my stomach starts to grumble in anticipation of whats gonna be at the station to eat.*

'So uh...did we ever get to order that pizza or are we gonna have to wait to eat haha'

'ima go with we have to re order haha'

'It's on me.'

'Haha Thanks Davis!'

(Wait till you get to the station before ordering that pizza, you may find something better there!)

'Thanks for the coffee' I say to Cory

*I drop off Cory on the apron of the fire station, then I pull around and park outside the Break room and walk in.*

*As I walk in, I unclip the radio extension mic from my shoulder*

'BLS-1 to COPA, clear of our medicals, available.'

'C'mon, please clear us!' I say to myself.

Ladder and squad 1 clear. (Stand by for after action report)
