Man on Radio Tower

"The EDRC will respond, However PEMS can send a Ambulance with a 15 min ETA." COPA Out.

*While, Im waiting for the drugs, I grab the Intubation kit and descide the patient is a 8.5 tube, get it and check the bulb, and then grab a 3 sized curved blade.*
'Lt. the sooner we get him to a Level 1 trauma center the better.'

"10-4 thanks."
'Cory, do you have a warm IV in there..or something? We gotta keep him stable long enough. 15 minutes. I'll be back.'
*I go over to the other side onto the drivers side and turn off all spotlights. I then walk into the radio building.*
'Hello! ERTFD!, Is anyone here?'
*A guy comes up from the back room.*
Is he down?
'Yeah, we're treating him now.'
Ok good. That B****** came back after I fired him yesterday. Probably pissed. Don't know why he climbed the tower though.
'Alright, did he come in here?'
Yea, really quickly though. Started yelling then left. Never heard about him until the radio messed up. Then I called you.
'Alright sir. I'm going to need to get you're info here. What's your name?'

*I write all his info down on a post it and put it in my pocket.*
"Ladder 1 to COPA, can we get PPD down here?"
*I walk back to the ladder then Cory tells me about level 1 trauma.
'Can't do much for him until an ambo gets here.'
'Hold tight sir, you're gonna be alright.' I say to the man.

(A GCS of 7 he's abtunded, he can't understand you )
'If you can get me the drugs I need that would be great, I don't want to give him the chance...'
*As I start to finish the sentance, the pt. starts to get up and tries to rip his IVs out mummbling unrecongizibly.*
'Crap grab me the Versed, assp, its the vial with the green top.'

'Alright'
*I dig into the ALS bag and hand Cory the vile. I get a cloth and wrap the mans hand up (I forgot the technical term for this).*

*I come around by his hands and keep an eye on them even though they are tied*

"Rodger that Hannahan" I say through the ERT Radio

"ERT to any available PPD unit. Requesting you to respond to the radio tower to assist ERT FD" I say through the City Radio.

"PPD is on the way" COPA out.

*Draw up the Versed and administer 2.5 mg IV. The pt. relaxes, and stops fighting.*
'Alright, just put his arms under the backboard straps.'
*I reach into my bag and get out the Succs and the atropine. I administer the 15 mg Succs, grab the laryngoscope and visualize the vocal cords and pass the tube. I then check for breath sounds bilaterally and over the epigastrim. I secure the tube and give the BVM off to Lt.*
'I want you to squezze count to 3 one hundred and then squezze again, and repeat.'
*I grab the atropine and give 0.5 mg IV rapidly pushed. I check the EKG and find that his rate has increased to 60.*
'Alright, Lt. I got Sinus on the monitor, but he's showing cushings triad.'

*I get up and get my radio from off of my shoulder.*
"Ladder 1 to COPA, 10-4 thanks."
'Hopefully they come in two's.''
*I hear the siren of the PPD and see their car turning the corner. The two officers get out and walk over to us after talking on the radio.*
Hey, what's going on?
'Nothing man. Uhhh.. rescued this guy off of the radio tower and is acting crazy right now. I got the info of the store manger inside of the building and he said the guy was fired yesterday, cam back today and started yelling.'
Alright, I could help ya.
*I see one PPD officer go into the building and the other PPD officer help Jessica. I then hear the PEMS ambulance siren and hope they're close.*

*I put hte guys arms under the backboard straps at his sides with the help of the officer and make sure they are secure so he cant move them or losen them up and get them out*

"Hutchison to EDRC and COPA,COPA please relay to PEMS, we have a 30 y/o male, GCS of 3, after RSI, vital are pulse 60 regular, with Sinus rythm on the monitor, B/P is climbing 190/110, Resp. 30 and irregular, possible kussmals respriations. We have 2 IVs est. in L and R AC both 14 ga. Succs, Atropine, and Versed have been given, as well as a 8.5 tube has been placed successfully. I need an ETA on PEMS."
*Without waiting for COPA to respond, I check the lines and slow them down to a TKO. I through a blanket over the patient.*

(GCS of 3? he's out cold haha. Also, lets go slow. Let EDRC respond too)
*I hear the sirens getting closer now from both sides and remember that the EDRC is responding too.*

'Good, he needs to go or he won't make it.'
*I grab Labetalol, a beta blocker, out of the drug box and give 20 mg IV slow push then recheck the vitals*
'Alright, B/Ps down to 170/100, but I won't get it much lower.'

'Alright good.'
*As crowds start to come up, the other PPD officer comes out just in time. He then gets in front of the people and starts to tell them all to move back.*

*comes over to the Lt.*
'Hope they get here soon...this crowds gettin pretty big'

'Yeah, everyone likes to see someone get hurt.'
*I look over at Cory then walk back over to him.*
'You still need help?'

I walk around the Ladder. The stick wont be needed any more so I set it in its resting spot. I walk around back and check for people when I put up the out riggers. Finaly I walk around and pick up the pads.
((The Ladder seems packed. I really have been gone awhile :/))

*I look around noticing the crowd getting bigger and bigger*
'Hey Lieu how many people do you think are standing there?

I think to myself that its a bit cold out. I watch as the snow falling illuminated by the trucks lights, I see Cory working on the vic and hear sirens of the approaching vehicles.

*I look over at the crowd of people and see them trying to look over here..*
'Ya know, I don't know and don't care.'
*I see one of the officers shouting into his walkie talkie.*
'I think more is coming. They have their hands full.'

*I walk over to Jack as the snow lightly falls around us*
'Hey, my names Jessica, or Jess if you like, I'm the new Firefighter/SAR on the ladder. Seeing as how we have...well...not that its really down time but theres not really anything for us to do except wait for the medics to arrive'

*I recheck the vitals*
'This is not good. Vitals are decreasing, can I need the pt. hyperventilated at 20 rpms (resp. per min). That will blow off the excess CO2 to get the B/P back down and the Heart Rate up.'

'You need the BVM for that?'

(I thought you had been baging the whole time! Look back it was right after I tube him.)

(Woops sorry, forgot haha. Aren't 1 every 3 seconds going up to 20 puffs a minute? We were already at 20.)
*I start going a little faster up to 20 rpms per second and see the man's chest rise a bit.*

( sorry guys I would of been on eralier but we had an mva this morning so I slept in afterwords)

* I walk over to Jessica and Jack and await orders with them*

*I get up from bvming the guy and point to Tim.*
'Tim, help Cory with the BVM. Bruce, stay with Cory and get him whatever he needs. Jess, check over all of the equipment for me and tell me what's missing.'
*I quickly hand the clipboard to Jess.*
'Alright Cory, they're coming.'
*A PEMS ambulance comes around the corner and an EMT and paramedic pop out.*

* I take over Hannahan's job and join cory with the bvm*

((sorry i was on a 24 hour duty crew. can some one get me up to speed?))

(good afternoon) *I start checking all the equipment*

((nice to meet you jessica))
'cory what do you need me to do?'

*I keep checking the equipment checking it off on the clipboard as I go, I also start putting back any of the equpment that was used that we don't need anymore*

* I pull up in the EDRC and shut down the lights and siren. I grab the trauma kit and drug bag, and sprint over to Hutchison.
'Sorry, got held up at the hospital. Current stats?' * I ask as I put on a pair of rubber gloves.

((Sorry guys, was at a birthday party last night and didn't have computer access.))

*I see the PEMS ambulance pull and think to my self thank God! I recheck vitals I get up, making sure everyone that is taking care of the patient is doing what their supposed to be doing, and I walk over to the Medic.*
'Alright sir, we have an approximately 30 y/o male, pt. has a GCS of 3 maintained by succynialcoline(sp?) and Versed. Pt. has pulse of 40, resp. of 20 by BVM, B/P is 170/100, monitor reads Sinus bradycardia upon palpation the pt. has a stairstep of C3-C4, and PMS is negitive in all 4 extremities. We have 2 IVs est., one in each AC both are 14 ga, and an 8.5 ET has been placed successfully. Pt. has been give Labetalol for HTN, and Atropine for Bradycardia.'
*The PEMS medic nods in aproval.*
'Ok, I'm gonna need some extra hands in the back, how about you and an the guy bagging.'
*I grab my mic and press the PTT*
"Hutchison to Hannahan, PEMS is requesting my self and Tim assist with transport, and we're gonna need everybody to help lift him, also notify COPA of condition, will need a level 1 trauma alert."
*As I finsih transmiting, I notice that the EMT from PEMS is getting their cot out and moving it towards the patient. I go over and croch down taking the head, moving Tim off to the side a bit.*

*I finish checking the equipment and with nothing missing from the truck besides what we are using I walk back over and hand John his clipboard back and then walk over to cory*
'Ready when you are'

*i reach down and grab a corner on the bord. and wait for the command to lift*

'Alrigh guys lets get ready to lift.'
*I quickly get hold of my radio and press the PTT button and wait a second.*
"Ladder 1 to COPA, PEMS is on scene and needs assistance with transport. I've authorized for Cory and Tim to ride along to the hospital. Can you alert the receiving hospital that this will be a level 1 trauma alert? over"
*I put my radio back on my clip and get a piece of the backboard.*
'Alright here we go, 1..2..3.'

*i lift with the rest of the crew and set the back bord on the cot*

*I lift with the rest of the crew carefully helping to set the backboard onto the cot*

*We strap the guy down after putting him on the stretcher and get ready to load him into the ambulance. My heart races and for some reason, I feel bad for the guy.'shouldn't have climbed up there in the first place.' I thought to myself.*
'Alright, lets get him into the ambulance.'

*As they start to roll him to the ambulance, I check all of the IVs and the ETT to make sure that they weren't dislogded. I look up and notice Kaizer's vehicle pulling up*
"Hutchison to EDRC, We have patient preped for transport and are preparing to load in to PEMS' ambulance. Here is what we have an approximately 30 y/o male, pt. has a GCS of 3 maintained by succynialcoline(sp?) and Versed. *Once we get to the ambulance, I go around and put the step so the stretcher can be put in. I anxiously look back at the man and see he's still not responding.*
'Alright, Cory, when you're ready.'
*I get on the side of the stretcher.*
'Jess get on the other side and Bruce, get the wheels when we lift.'

*i bend down and grab the wheels*

*I get on the other side of the stretcher*

*As we lift the stretcher into the ambulance gracefully as if the patient was riding on a calm wave, I couldn't help but think about how cold he might be. I rememeber he might be drunk then just forget about my feeling.*
'Alright Kaizer, you going in?'
*I close both doors after making sure the stretcher is locked in and close both doors. I then use my foot to put the step down.*
'Alright, he should be alright hopefully. Jess you have my clipboard? Bruce, how ya feeling?'
*I look up and see that the crowds are dispersing. and feel good inside.*
"Ladder 1 to COPA, victim is in the ambulance now. Advise if Pumper 11 or Squad 1 needs us please."
'Report time.' I say while clapping my hands.

*i walk over and climb in the drivers seat of the ladder and adjust the seat*
'hannahan when ever your ready'

*I climb into the ladder and take my helmet off*
